Comparison of renal elastography values ​​in children with type 1 diabetes mellitus and healthy children

Abstract (EN)

Shear wave velocities measured by shearwave elastography, a non-invasive method, in children with Type 1 Diabetes Mellitus without diabetic nephropathy. We aimed to compare (SWV) values with healthy children. According to the diagnostic criteria published by the American Diabetes Association,Type 1 DM diagnosis (HbA1c > 6.5%, fasting blood glucose ≥ 126 mg/dl, postprandial blood glucose ≥ 200 mg/dl) was younger than 18 years old, but Diabetic Nephropathy (DN) Undiagnosed 30 patients with normoalbuminuric, creatinine values, and glomerular filtration rate (eGFR) within normal limits for age and 30 healthy children with normal clinical laboratory findings were included. Shear wave elastography was performed on all patients and the healthy group. Values in meters/second were obtained with the help of ROI from the upper, middle and lower zones of both kidney parenchyma. A total of nine SWV measurements were made from each kidney, three from each zone, and the mean of these measurements for both kidneys was calculated. There was no statistically significant difference between the patient and control groups in terms of age and gender. (p>0.05). In the healthy group, parenchymal SWV values measured from the right and left kidneys were found to be 1.91 ± 0.38 m/s, 2.14 ± 0.41 m/s, with the mean of right and left. In the patient group, mean parenchymal SWV values measured from the right and left kidneys were found to be 1.99 ± 0.24 m/s, 2.06 ± 0.34 m/s, as right and left. According to these results, there was no statistically significant difference between the patient group and the healthy group in the mean SWV values of the right and left kidneys. (p>0.05). Accordingly, elastography values of Type 1 DM patients without DN and healthy control group were found to be similar in our study. In Type 1 DM, which is a chronic disease, the development of DN, which has a chronic complication, begins within an average of 5-15 years after diagnosis. In our study, we did not find any difference in elastography values in early-stage diabetic children with the normal relatively non-patient population. These results show us that the results of elastography can be important. We think that elastography can be used in the follow-up of the disease in the early stage and in the differentiation of advanced stages, in order to determine whether the kidneys are affected or not and to take the necessary precautions. However, this study should continue in patients with advanced disease. We think that our study will lead to more comprehensive and controlled studies on renal elastography in the future.
