Abstract (EN)
FNAB (Fine-needle aspiration biopsy) is a very important in evaluation of thyroid gland diseases. The suspicion of follicular neoplasm have been considered a diagnostic gray zone in thyroid FNAB. The lesions suspected follicular neoplasm as a result of FNAB; represent the follicular adenoma, adenomatous hyperplasia, follicular carcinoma or follicular variant of papillary carcinoma. The purpose of this study is to evaluate of the parameters may help in prediction of malignant disease with preoperative assessment in patients suspected follicular neoplasm as a result of FNAB.CUTF 152 patients underwent thyroidectomy, included to the study with the suspicion of follicular neoplasm in Department of General Surgery. Patients were evaluated in according to the relationship between malignancy seen with the vascularization characteristics, the peripheral halo presence around the nodule and demographic characteristics, serum TSH (thyroid stimulating hormone) levels, the number of nodules on ultrasonography, diameter, nodule boundary scheme, intranodulaire calcification and vascularization characteristics.127 of patients were female, 25 of them were male. The average age were 48.14. Malign disease was detected in 28 patients (18.4%). To be irregular of nodule boundary, intranodulaire calcification or increased vascularization and the presence of peripheral halo around the nodule was significantly found in high grade for malign lesions.Malign disease is detected in approximately 20% of patients suspected follicular neoplasm as a result of FNAB. The remaining 80% patients are operated due to benign disease. Because of not-evaluating the capsule invasion in frozen study, this are inadequate in the differential diagnosis of lesions. In prediction of malign disease, the detection of adjuvant diagnostic methods is high the value of evidence is to be changed the operation plan or even the operation indications.It has been ascertained that the possibility of malign disease increased by reason of being irregular of nodule boundary, the presence of calcification in nodule, to increase the intranodulaire vascularization and the presence of peripheral halo in conclusion of this study. The risk index to be created in consideration of datas to be obtained in more extensive studies may be beneficial in surgical management of thyroid nodules suspected follicular neoplasm.Keywords: Suspicion of Follicular neoplasm, thyroid FNAB
