Abstract (EN)
Thyroid nodules are an important health problem today. Fine-needle aspiration biopsy (FNAB) for diagnosis of thyroid nodules is evaluated with the Bethesda classification. Evaluation of thyroid nodules to exclude thyroid cancer includes FNAB, depending on clinical and ultrasound features. In the large series of patients classified according to the Bethesda System in the guideline of the American Thyroid Association (ATA), non-diagnostic (non-diagnostic) samples constitute 2- 16% of all FNAB samples. The aim of our study; To determine the diagnosis rates of ND in patients who underwent FNAB for thyroid nodules, to investigate the factors affecting the diagnosis of thyroid cancer in patients with ND as a result of FNAB, to determine the malignancy rates in patients with ND cytological results and to compare them with the literature. Patients who underwent thyroid ultrasound (USG)-guided thyroid FNAB between January 2011 and January 2020 in Tokat Gaziosmanpaşa University Research and Application Hospital and whose result was ND were included in this study. Patients with FNABs with ND were retrospectively scanned in the automation system of our hospital. In our study, our malignancy rate was 2.7% (23/846) in thyroid nodules with ND FNAB at baseline. Of the 846 patients with cytology diagnosis of ND included in our study, 114 (13.4%) were operated. The pathology result of 23 (20.1%) of 114 patients who underwent surgery was malignant. In our study, T4, Age, Hypothyroid Thyroid Function and Microcalcification were found to be statistically significant on malignancy (P=0.003; P<0.001; P=0.031; P<0.001, respectively). As a result of our study, the presence of microcalcification, low age, low T4 value and hypothyroidism increase the risk of malignancy in patients with ND FNAB cytology results. Clinicians can consider these findings as a complementary marker for patients with ND on FNAB who repeat these findings, for patient management protocols, and when determining the operation decision.
