Evaluation of blood flow parameters with triplex doppler ultrasonography in the thyrotoxicosis

Abstract (EN)

Purpose: Evaluation of the usefulness and efficiency in the differantiate diognosis of blood flow parameters in the superior thyroid artery and common carotid artery with triplex Doppler ultrasonography in the thyrotoxicosis casesMaterial and method: Study group was composed of totaly 72 cases. 18 of the cases were healthy control individual group and the 54 of the cases were the patient that diagnosed with standard methods as Basedow-Graves? disease (BGD), Hashimoto thyroiditis (HT) and multinodular goitre (MNG).Thyroid gland blood flow paramaters in the case was evaluated with triplex Doppler ultrasonography. Besides of blood flow paramaters as peaksystolic velocity (PSV), enddiastolic velocity (EDV), resistive index (RI), also resistive index ratio (RIR), peaksystolic velocity ratio (PSVR) and enddiastolic velocity ratio (EDVR) were obtained from both superior tiroid artery and common carotid artery. .Nonparamaters tests (Kruskal-Wallis, Mann-Whitney U-test) for the group means , and Pearson Chi-Square and Fisher?s tests for comparison of independent group ratios were used for statisical analysis.Results: Study group were divided into four main group by using standart diagnostic methods as follows: group 1:healthy control individual group 2:patients have Hashimoto Thyroiditis(n:27 36.5%) group 3:patients have Basedow Graves diseases( n: 17 24.3 % ) group 4: patients have multinodular goitre( n: 10 13.5%)The Hashimoto Thyroiditis group were classified subgroups such as euthyroid ( group 2A n:13 50%), hypothyroid (grou 2B n:7 25%) and hyperthyroid (group 2C n:7 25%) respectively.In the evaluation of STA PSV, STA EDV, CCA PSV, CCA EDV, PSVR and EDVR values between four main groups only BGH group had significant difference from other groups (p<0.05/4=0.0125). In subgroup of Hashimato Thyroiditis only between hypothoroiditis and hyperthroiditis subgroups STA, EDV and RI values had detected significant.Between BGH and hyperthyroid Hashiomato thyroiditis groups STA PSV, STA EDV, PSVR and EDVR values had significant difference (p<0.05/3=0.017). Significant difference was detected between hyperthyroid Hashimato thyroiditis and control group only for STA EDV value.Conclusions: We consider that STA PSV, STA EDV, PSVR and EDVR values could be used with high sensitivity, specifty and high positive and negative prediction in thyrotoxicosis cases that clinically difficult to diagnose and in interim case that both RDUS and B mode thrasonography patterns overlap.
