Evaluation of the patients with patent ductus arteriosus closed by transcatheter method

Özet (EN)

The rapid advances in technology result in the development of variety of devices with the ease of their use and make possible the transcatheter closure of different PDA types. In this retrospective study we compared the results, effectiveness and complications of the devices used in PDA closure in our center and their superiority to each other.The ages of total 392 patients whose PDAs? were closed via transcatheter method ranged from 1 month to 49 years, and mean age was 4.9 + 5.2 years and median age was three years. Of the patients, female / male ratio was found as 1.8. Concerning the mean age of the patients, ADO II device was mostly used in little age group more, while, Flipper coil (Cook) was used in the oldest age group (p = 0.001). It was confirmed that the average body weight of patients for whom ADO II device was used significantly lower than the average body weight of the patients for whom Flipper coil (Cook) was used (p=0.001).For the patients with pulmonary hypertension, ADO I device was significantly used more (p = 0.001). ADO II device was more frequently used in patients with Type A duct (33.6%). ADO I device was used more often in patients with the widest Duct diameter (D1), Gianturco coil was used more significantly in patients with the narrowest diameter of D1 (p=0.001).Six months after the procedure, residual shunt was found in 2.6% of the patients. Residual shunt most frequently observed in patients who were used NOC device was found 46.7% after the procedure, 31.2% 24 hours later and 9% six months later. Echocardiography after the procedure showed increased velocity and/or stenosis in descending aorta (3.2%) and in left pulmonary artery (2.1%) of the patients.As a result, the patient and device selection are important for transcatheter closure. Besides the balance between the patient age, patient weight, duct dimensions and the size of the selected device must be evaluated carefully.Key words: Device selection, Patent ductus arteriosus, Transcatheter closure
