Evaluation of relationship between ligamentous and kyphosis angle after treatment of traumatic vertebrae injury

PURPOSE: In this study, traumatic thoracolumbar kyphotic angulations which developed after trauma were measured and evaluated pre and postoperatively. We aimed to determine comparison of the efficacy of surgery and in which patients anterior approach should be done firstly.MATERIALS AND METHOD: Twenty-nine patients who were operated for spine injury between January 2007- January 2012 in Celal Bayar University School of Medicine Department of Neurosurgery evaluated in this study after obtaining approval from the local ethics committee . Nineteen of 29 were male and 10 of 29 were female. Preoperative and postoperative x-rays, computed tomography and magnetic resonance images were evaluated.RESULTS: Nineteen of 29 were male and 10 of 29 were female. Three of 29 patients had thoracic 11 fracture, 5 of 29 patients had thoracic 12 fracture, 8 of 29 patients had lumbar 1 fracture and other 7 of 29 patients had lumbar 2 fracture. While 18 of 29 patients treated with posterior surgical approach, 11 of 29 patients treated with combine anterior and posterior surgical approach. There was no statistically significance between two groups in terms of age. Total follow-up period of the patients was found to be 20-62 months. Follow-up time was no statistically significance between only posterior surgery group and combine anterior and posterior surgery group. The mean time for only posterior surgery group and combine anterior and posterior surgery group were consequtively (13,67-32,00) The distribution of patients according to their BMI there was no statistically significant difference between the two surgical groups. Sagittal index were compared by using lateral plain radiographs of patients before and after surgery. Changes in the direction of improvement was found in both groups but there was no statistically significant difference between the groups.DISCUSSION: Basically by using the load sharing theory, in patients with non-serious front column injury we preferred only posterior surgery and the results provided by the amendment values of SI and ASIA show that the right-fiction. Combine surgery group is preferably applied to front column injury presented with high kyphotic angulation. In Combined surgery group with supportion of front column provided by the SI and ASIA maintained throughout follow-up.Surgical complications which published in the literature for this method were not seen in our clinic. Although this method requires surgical experience may be preferred in patients who require front column support.
