Abstract (EN)
Background/Objective: Arthroscopic repair of a Bankart lesion which is an injury of the anterior glenoid labrum of the shoulder due to taumatic shoulder dislocation has become a standard treatment modality today. In the literature, successful results have been reported for the arthroscopic repair of Bankart lesions using two or more portals.In this study,we present our results of arthroscopic Bankart repair through single anterior portal. Methods: Between 2014 and 2018 ,a total of 53 patients (49 males,4,females) 54shoulder(one patient was operated on two shoulders at different times) who underwent arthroscopic Bankart repair through single anterior portal by a single surgeon for reccurrent traumatic shoulder dislocation were included in this study.Patients with accompanying bone defects, without traumatic dislocation were excluded.All patients underwent a detailed medical history,instability tests,direct X-ray and magnetic resonance imaging preoperatively.Under general anesthesia in the lateral decubitus position,standard posterior portal was created for the insertion of the epidural needle and the anterior edge of the glenoid was reached at the widest angle.A single anterior portal was used and repair was performed.All patients were scheduled for rehabilitation after a four-week immobilization period.The Rowe and Oxford scales were used for the pre- and postoperative evaluation. Results:The mean age of the patients was 28(range ,16 to 66) years.The mean folow-up was 26 (range, 9 to 60) months.The lesion was localized in the right side in 34 patients and in the left side in 20 patients.The mean time from the first dislocation to surgery was 45,8 (range,1 to 288) months. The mean number of dislocation on admission was 17,5 (range, 1 to 60),while the mean number of anchorsused during surgery was 3 (range,1to 5). The mean duration of surgery was 44,6 (range,32 to 70)minutes.Postoperatively, the apprehension test result was positive in three patients.No reccurrent dislocation was seen in these patients and no additional treatment was applied.None of the patients required reduction. The mean preoperative Rowe score was 12(range,0 to 45),while the mean postoperative Rowe score at the final follow-up was 93,1 (range,70 to 100) (p=0,001). The mean preoperative Oxford score was 18,5 (range, 0 to 29),while the mean postoperative Oxford score at the final follow-up visit was 45,7 (range, 35 to 49)(p=0,001). Conclusion: Our study results suggest that arthroscopic repair of a Bankart lesion through single anterior portal is safe, successful and less invasive in the treatment of traumatic shoulder dislocation.
