Abstract (EN)

EVALUATION OF THE EFFECT OF PLATELET MARKERS ON THE AMOUNT OF INTRAOPERATIVE BLEEDING IN VERTEBRA SURGERY AIM: One of the most important steps in perioperative bleeding management is the predetermination of the patient at risk. In vertebral surgeries where major bleeding can be encountered, there is no study in the literature examining platelet markers in terms of predictivity in intraoperative bleeding to date. In this study; We aimed to investigate the efficacy of preoperative mean platelet volume (MPV), platelet distribution width (PDW), plateletcrit (PCT), mean platelet volume lymphocyte count ratio (MPVLR) and platelet and lymphocyte count ratio (PLR) values in predicting the amount of intraoperative bleeding in patients who will undergo vertebral surgery. METHOD: After the approval of the ethics committee, the patients who were planned for vertebral surgery by the neurosurgeon in our hospital were determined. Demographic data such as gender, age, ASA score and body mass index (BMI) of the patients included in the study; MPV, PDW, PCT, MPVLR and PLR values were recorded from the preoperative hemogram. Total surgery time, vertebral transpedicular screw and laminectomy level numbers were recorded. The amount of intraoperative bleeding was obtained by making necessary calculations on the amount of blood accumulated in the aspirator during the operation and the amount of blood in sponge. RESULTS: Of the 63 patients included in the study, 60.3% (n=38) were female and 39.7% (n=25) were male. The mean age of the patients was 54.7 ± 11 years, and the mean BMI was 29.6 ± 4.6. No correlation was found between gender, age, platelet markers (MPV, PDW, PCT, MPVLR and PLR values) and the amount of bleeding. A significantly high correlation was found between BMI, duration of surgery, number of transpedicular screwing segments and amount of bleeding. CONCLUSION: As a result; While there was no correlation between the amount of intraoperative bleeding and the patient's gender, age, MPV, PDW, PCT, MPVLR and PLR values; We found that the amount of bleeding increased as the patient's BMI increased, the duration of surgery increased, and the number of transpedicular screw segments applied increased.
