Abstract (EN)

Aim:It is frequent that the togetherness of congenital heart diseases and Down syndrome. In children with down syndrome most common are congenital heart disease 36% with Atrioventricular septal defect (AVSD). It is known immune deficiency in children with down syndrome. In the studies conducted on this subject especially the increasing in IL-4,IL-10, the decreasing in TNF-alfa and IL-6 clarify the causes of immune deficiency. Also, the complete AVSD is the shunt pathology which is from left to right. Accordingly it is conginetal heart disease in which increase lung bleed. Because of the togetherness of Down syndrome and Complete AVSD, it seen both deficiency of immune system and lung bleed because of increasing AVSD as a result growth, retardation lung infection. Material and Method: We purposed to research whether down syndrome effect to morbidity and mortality or not in postoperative process of Complete AVSD's patients which is full corrected. Therefore in Çukurova University, Faculty of Medicine, In the Department of Cardiovascular Surgery between the dates January 2011 and January 2014, it was compared according to prospectively postopreative process, morbidity and mortality, 30 patients totally, from which 20 patients (Group A) with down syndrome and Atrioventicular septal defect,who underwent total surgical correction by open heart surgery method and 10 (Group B) of them are patients without down syndrome. Rsults:In postoperative process; it is seen that in group A, inotropic agent requirements, the long stay duration in ventilator support and intensive care, come across with pulmonary problems, to which cause atelectasis and infection more and the mortality is higher in this group. It is not determined significant difference between group A and B, according to drainage, blood products replacement therapy, postoperative echo findings. Conclusion:In our study, it proved that the postoperative process of total corrected Complete AVSD's facts with Down syndrome are more problematic, the morbidity and mortality are higher.
