According to the Guideline which has published with regard to Injury Criminals which are expressed in Turkish Penal Law, 1-Year Cost Analysis of Cases that are sent to Forensic Medicine Outpatient Clinic for Submission of the Final Report, Submitting a Temporary Report instead of being able to be Submitted a Final Report in the Medical Organization that is applied, Zonguldak Bülent Ecevit University, Faculty of Medicine, Forensic Medicine Specialization Thesis, Zonguldak, 2019. Aim; In our study, it is aimed to determine the sociodemographic characteristics of the cases, to determine the factors of the physicians' decisions on a temporary report, to reveal the reasons of deficiencies and mistakes in the submitted temporary reports, and – the most important- to calculate the cost of these deficiencies and mistakes for the government and people by scanning backdate the cases applied to Forensic Medicine Outpatient Clinic to take only the final report, submitting a temporary report instead of being able to be submitted a final report according to the Guideline for Forensic Reports that will organize within the frame of Turkish Penal Law no.5237 by the practitioner or specialist emergency physicians who are frequently encountered forensic cases or by the specialist physicians who decide to hospitalize a patient. Material and Method: It was evaluated that the general forensic examination reports, which are filled in the emergency services, of 395 cases that are sent to Zonguldak Bülent Ecevit University, Faculty of Medicine, Department of Forensic Medicine between the 1st January in 2018 and the 31st December in 2018 for submitted a final report under the guidance of the Guideline of Evaluations the Injury Criminals which are expressed in Turkish Penal Law in terms of Forensics, having regard to the results of the examinations and investigations and –if patients were hospitalized- the results of all the examinations and investigations in the department they were hospitalized. Forensic cases were divided into 3 groups as ones that were submitted a temporary report instead of being able to be submitted a final report by the physician who was applied for the first time, ones that were submitted a temporary report instead of being able to be submitted a final report by the physician who is in a department that the patient was hospitalized or in a section the patient was sent, and ones that cannot be possible to be submitted a final report if that one doesn't arrive the Forensic Medicine Outpatient Clinic. It was calculated the average cost for each cases by answering some questions such as the cases in the 1st and 2nd group whether they arrive only for a final report or for also the aim of treatment in the date of arriving the hospital for a forensic report, whether they are able to work or not, occupations of those who are able to, residences of all the cases, whether their relatives accompany them, and it was tried to be calculated the total cost for Turkey in general and the city Zonguldak and its towns on the basis of one case's average cost. In addition, the rate of forensic cases in all patients admitted to the adult emergency and paediatric emergency departments of Zonguldak Bülent Ecevit University Medical Faculty was determined through the information system used by our hospital. 200 patients (150 patients with adult emergency, 50 patients with paediatric emergency) were selected from 2204 forensic cases by using random sampling method. To what extent those 200 cases were submitted a temporary report and to what extent they were submitted a final report were investigated and the determined data were evaluated in the light of the literature. In the calculation, it was accepted that 4 out of every 100 cases that applied to emergency departments were forensic cases and 90 of every 100 forensic cases were submitted a temporary report. Statistical analysis was performed using SPSS 19.0 software. Findings: It was seemed that 365 out of 395 cases that were submitted final reports in the Forensic Medicine Outpatient Clinic were able to be submitted a final report before applying to the Forensic Medicine Outpatient Clinic. It was determined that 2204 forensic cases (3.84%) applied to Zonguldak Bülent Ecevit University adult and paediatric emergency services in 2018. It was found out that cases in the group of 0-18 ages, poisoning cases in any age groups, ones who leave from the emergency service for some reasons (referring, hospitalizing, or voluntarily) except for being discharged and all injuring except for simple soft tissue injuries (ecchymosis, abrasion, oedema) were submitted temporary reports significantly more than other cases. It has been thought that on average 100 million people apply to emergency services in a year in Turkey, 4 out of every 100 cases applied to emergency services are forensic cases, on average 90 percent of the forensic cases was submitted temporary reports. Results: It was found out that the cost of each of the cases themselves and of the government is 155.6 Turkish Liras in average in the calculations made, the total cost of the cases that are submitted temporary reports instead of being able to be submitted final reports in the departments they are hospitalized or in emergency services in Turkey in the year of 2018 is 520 Million Turkish Liras in average( 3.337 million people x 155.6 Turkish Liras) , and it is 3.8 Turkish Liras in average in Zonguldak and its towns. The cases' final reports must be submitted to the forensic cases that are discharged after their treatments in the emergency service by the physician in that emergency service, to the forensic cases that are hospitalized in any department or sent – if they were submitted a temporary report in the first emergency service applied – by the physician who works in the department where the cases are sent or hospitalized when they discharge. Also the importance given to forensic medicine education in medical faculties should be increased, practical forensic report writing trainings should be performed especially in pre-graduation internship period. In addition, we think that performing practical forensic report training periodically to the emergency physicians working in the field and other specialist physicians at certain intervals will decrease the mistakes made in forensic report writing.
