Evaluation of medial opening wedge osteotomy results at varus knee gonarthrosis clinically and radiologically

Objective: Our aims were to evaluate short and long term results of osteotomy, and how patellar tendon length and angle alternations ( femorotibial angle, tibial slope angle like..) effects results clinically and radiologically for the patients who were treated for medial compartment osteoarthritis by medial opening wedge osteotomy(MOWO) in our clinic.Patients and methods: We treated 32 patients with high tibial osteotomy and 25 of them included our study. 26 knees of 25 patients investigated in this study. All patients are women and their average age were 57,04 years ( min. 46- max. 69). Average follow up period of 25 patients were 25,4 months (min. 8- max. 69 months). Radiologically femorotibial angle, femoral condyle - femoral shaft angle, femoral condyle - tibial plateau angle, tibial plateau ? tibial shaft angle, tibial slope angle and patellar index ratios (Insall - Salvati, Blackburne - Peel, Caton index) were measured on preoperative, postoperative(3.month) and last follow up radiographs. Arthrosis were evaluated arthroscopically and radiologically according to Outerbridge classification and Ahlback classification. HSS, WOMAC (Western Ontario and McMaster Universities Index of Osteoarthritis) and VAS (Visual Analogue Scale) scoring systems were used for the clinical evaluation.Results: Changes of femorotibial angle at the MOWO done patients were statistically significant and this changes effects clinical scores positively. As time goes by correction loss occured at the femorotibial angle and this loss effects HSS scores negatively. Postoperatively, patellar index ratios were decreased and tibial slope angle were increased significantly. However, these changes were not effects HSS scores statistically. Preoperative osteoarthritis severity and age were not effects our patients clinical scores. HSS, WOMAC and VAS scores increased significantly in our patients.Conclusion: We believe that medial opening wedge osteotomy technique is preferrable for the patients who are suitable for osteotomy criterias and have varus gonarthrosis.Keywords: High tibial osteotomy, medial open wedge osteotomy, radiologic evaluation, clinical evaluation.
