Abstract (EN)
Purpose: West syndrome (WS) s recognzed as an epleptc syndrome of nfancy, characterzed by a trad of spasms, a specfc electroencephalographc fndng called hypsarrhythma pattern, and stagnaton or regresson of psychomotor development. In our study, bran parcellaton was performed on dffuson tensor mages of specfc whte matter tracts (superor paretal lobule, nferor frontal lobule, mddle occptal lobule, cngulum, hppocampus, cerebellum, amygdala, thalamus, nferor fronto-occptal fascculus, superor fronto-occptal fascculus, uncnate fascculus, corpus callosum genu, body, splenum, cortcospnal tract, sagttal stratum, fornx/stra termnals) for both control and patent groups, and voxel, fractonal ansotropy (FA), and mean dffusvty (MD) parameters were evaluated. The am was to observe the subacute and chronc perod effects, dstnct from the early effects of West syndrome. Materals and Methods: Our study ncluded 30 patents aged 0-4 years dagnosed wth West syndrome (WS) and 29 patents dagnosed wth smple febrle sezures (SFS), who were followed up n the Pedatrc Neurology Clnc of Karadenz Techncal Unversty. Cranal magnetc resonance magng (MRI) mages of patents referred by the Pedatrc Neurology Clnc were retreved from the Pcture Archvng and Communcaton System (PACS) and analyzed. The dffuson tensor magng (DTI) method was used for the analyses. In the study, voxel, FA, and MD parameters were obtaned by performng bran parcellaton on dffuson tensor mages for both control and patent groups. Fndngs: The FA values of the blateral superor paretal, blateral nferor frontal, blateral posteror thalamc radaton, blateral cngulum, blateral fornx/stra termnals, blateral superor longtudnal fascculus, blateral nferor fronto-occptal fascculus, blateral sagttal stratum, blateral genu of the corpus callosum, blateral body of the corpus callosum, blateral splenum of the corpus callosum, left mddle occptal, and left cortcospnal tract regons n patents wth West syndrome were found to be sgnfcantly lower than the FA values of patents wth smple febrle sezures. The MD values of the blateral superor paretal, blateral nferor frontal, blateral amygdala, blateral hppocampus, blateral posteror thalamc radaton, blateral superor longtudnal xvii fascculus, and blateral sagttal stratum, rght posteror thalamc radaton, left fornx/stra termnals, left cerebellum, rght uncnate fascculus, rght genu of the corpus callosum, rght body of the corpus callosum, rght splenum of the corpus callosum, and rght thalamus n patents wth West syndrome were sgnfcantly hgher than those n the control group. In patents wth West syndrome, the voxel values of the blateral superor paretal, nferor frontal, cortcospnal tract, posteror thalamc radaton, cngulum, superor longtudnal fascculus, sagttal stratum, genu of the corpus callosum, body of the corpus callosum, splenum of the corpus callosum, and thalamus were sgnfcantly decreased compared to the voxel values n patents wth smple febrle sezures. Addtonally, there was a marked decrease n the voxel values of the left mddle occptal and fornx/stra termnals and the rght amygdala. It was also determned that there was a decrease n FA values, an ncrease n MD values, and a reducton n voxel values n the blateral superor paretal, nferor frontal, superor longtudnal fascculus, sagttal stratum, and posteror thalamc radaton regons. Conclusons and Recommendatons: These results suggest that the whte matter ntegrty of the patents s mpared and that myelnaton s delayed. Varous factors such as axonal propertes, fber coherence, demyelnaton, whte matter maturaton, cortcal malformatons, and epleptc actvtes may cause abnormal fndngs n DTI. Addtonally, pathologcal processes that alter the mcrostructural envronment such as neuronal edema or cell shrnkage, changes n extracellular space, and loss of tssue organzaton are thought to potentally lead to altered dffuson and/or ansotropy. We beleve that our results, partcularly the anatomcal and functonal mparments n regons wth blateral FA and voxel reducton and MD ncrease, may shed lght on the deteroraton n vsual, audtory, and motor functons observed n patents. The fndngs of our study support prevous DTI studes that demonstrate eplepsy-assocated DTI abnormaltes, especally n regons showng reduced FA and ncreased MD values.
