Comparison of the effect over inflamatuary response of the new volatil anesthetics

Abstract (EN)

SUMMARYIn our study, we aimed to compare the effects of the volatil anesthetics desflurane andsevoflurane to the inflammatory responses in 2 l/m and 4,4 l/m flow rates respectively, inlaparoscopic colesystectomy patients with the same intraabdominal pressures (10-14 mmHg).In ASA I-II risk groups, 30 volantary patients who planned to be performed electivelaparoscopic colesystectomy were included the study. Premedication, the drugs for anesthesiainduction, the hydration therapy during the operation, analgesia after operation werestandardized. The patients were divided into four groups randomly; in Group 1(n=8) 50% O2,50% air, with 2 l/m flow rate 1 MAC desflurane; in Group 2 (n=7) 50% O2, 50% air, with 4l/m flow rate 1 MAC desflurane; in Group 3 (n=8) 50% O2, 50% air, with 2 l/m flow rate 1MAC sevoflurane and in Group 4 (n=7) 50% O2, 50% air, with 4 l/m flow rate 1 MACsevoflurane were used.Blood samples were collected three times from patients in preoperative period, 30.minutes after insuflation and postoperative 24. hours and serum IL-2, IL-4, IL-6, CRP andPCT levels were evaluated. Statistical analysis of all the results were performed by usingSPSS for Windows 10.0 version. We didn?t determine any significant difference between thegroups in demografic datas and hemodynamic findings. There was no statistically significantdifference when we compared the groups for IL 2 values (p>0.05). All the results were innormal range of value.In the group that we used 2 l/m flow rate desflurane, there were statistically significanthigh levels in IL-4, as an indicator of inflamation, when we compared with the other groups inpostoperative period. The levels of IL-6 which is a proinflammatory cytocin; CRP and PCTwhich are known as the most valuable markers to determine inflammatory responses werehigh in all groups. It was observed that in laparoscopic colesystectomy performed with 10-14mmHg pressure for pneumoperitonium; in postoperative period IL-6, CRP and PCT levels gethigher than the preoperative period which is statistically significant and the effects of volatilanesthetics desflurane and sevoflurane to the inflammatory responses in 2 l/m and 4,4 l/mflow rates were not different.In our study we observed that in postoperative immune resonse, the effects of desfluraneand sevoflurane, in 2 l/m and 4,4 l/m flow rates are not different. So in laparoscopiccolesystectomy low flow rate anesthesic methods should be used to get lowest costs.
