Abstract (EN)

Comparison Using Morphine and Remifentanil İnfusion As The Maintenance Anaesthesia İn Neonates. Aim: In our study we aimed to compare using morphine and remifentanil infusion as the maintenance anesthesia in neonates. Material ang Method: After the approval of Çukurova Universty Medicine Faculty Ethics Commitee and parents , 43 patients who are American Society of Anesthesiologists (ASA) class I-II ,aged 0-28 days and undergoing thoracotomy or laparotomy were taken in the study. Randomly patiens were parted in two equal groups. After 1mg/kg ketamine induction, 30 ? kg-1 unique dose morphine to Group I patients (n=20) and 0,25 ? kg-1min-1 remifentanil infusion to Group II patients (n=23) were applied. Electrocardiography, systolic and diastolic blood pressures, peripheral oxygen saturation ,end-tidal carbon dioxide and ,amount of urine values were observed continually. Induction, maintenance and awakening features were recorded.Results: Demographic properties of kle groups were similar (p>0.05). İn our studies hemodynamic ratios of patients with induction of 1mg kg-1 ketamin and begining induction and entubation were not different. Only one dose 30 mg kg-1 morphine was used after entubation to the newborn. During the operation heart rate (HR), sistolik arterial pressure (SAP), diastolik arterial pressure (DAP) ratios were similar to the preoperative ratios. Remifenthanil 0.25 ?g kg-1min-1 infusion was apply to the second group. Hemodynamical stabilities was secured in the second group and HR, SAP, DAP ratios were similiar to the morphine group. Extubation time in morphine group was 9.86±13.42 min in remiphentanil group was 19.95±28.83 min. Statistically there was not a sensible difference (p>0.05). Hospitalisation time in intensive care ünit at morphine group was 165.1 hours and remiphentanile group was 86.7 hours. Thisstudy provided that hospitalisaton time in intensive care unit is different in two groups but statistically there was not a sensible difference (p>0.05). Conclusion: Using morphine or remifentanil infusion as the maintenence anesthetic in neonates undergoing thoracotomy or laparotomy supplies enough hemodynamic stability and causes no difference between postoperative extubation times and intensive care unit staying times.
