Abstract (EN)
Introduction: In the intensive care unit (ICU), it is difficult for patients at risk of malnutrition to prolong their life and meet the critical burden. There is no consensus on the most appropriate method to set these limits. Traditional forecasts and assessments often provide limited information due to subjective and biased structures. This prospective purpose of use brought together the length of stay, length of stay on mechanical ventilator (MV) and death rates in ICU patients who are thought to be at risk of feeding by using 4 different malnutrition tools. Materials and methods: A suitable sample consisting of 74 patients with respiratory failure who were admitted to Inonu University Anesthesia and Internal Medicine ICU clinics was used. Patients were evaluated by calculating MNA, SGA, CONUT, NRS- 2002 scores within the first 24 hours of hospitalization. Demographic information, length of stay in MV and length of stay in ICU, mortality rate were calculated. Descriptive statistics were used to examine the number/ratio of risk categories; mean ± SD was used to summarize demographic and clinical variables. Results: The mean age of the patients was 70.00±16.78 (min-18-max=95). Of the patients, 45,9% were female and 54,1% were male, and 26 (35,1%) of them died. SGAC, MNA, CONUT, NRS 2002 were all statistically significant during ICU stay (p<0.001). In determining mortality, the test with the highest specificity (91,7%) when CONUT cutoff point ≥ 7 was determined as the test with the highest sensitivity (92,3%) when NRS 2002 cut-off point ≥ 3 was taken. Conclusion: In our study, we showed that the efficacy of CONUT and NRS 2002 in predicting ICU length of stay, IMV length of stay and mortality is better than MNA and SGA. We think that it will provide a more practical and objective approach in determining the relationship between malnutrition and mortality in ICU patients.
