Abstract (EN)
Objective: The nosocomial bloodstream infections (BSI?s) causes serious medical conditions at critically ill patients at Intensive Care Units (ICU?s). Besides numerous chemotherapeutics, these critically ill patients are exposed to several invasive procedures. In this study, we aimed to emphasize the additional cost; prolonged hospital stay time; additional mortality rates caused by the central venous catheter (CVC) related primary BSI?s.Method: We designed a case control study which assesses the incidence of BSI?s; additional cost; prolonged hospital stay time; additional mortality rates; survival and the evaluation of risk factors that causes BSI?s between March 2006 to March 2008.Results: We calculate the cumulative incidence of nosocomial BSI?s is 13,8 and the incidence density as 2,01. The additional cost attributed to BSI?s is calculated as 19902,44 $, t: 5,579, p=0.000. The additional total cost of antibiotics (that used for BSI?s) is 68850,3 $, and daily cast of antibiotics is calculated as 4076 $, t:9,28 p= 0,00. Average of 15.61 day (t=4.00 p= 0.00) prolonged hospital stay is monitored for the BSI developed patients. We find the additional fatality rate attributed to the BSI?s is 15.2%. CVC exposure time [ ( exp (B)) 0,837 (%95 CI=0,749- 0,936) ] and the time period of feeding application [ (exp (B)) 0,841 ( %95 CI=0,781- 0,906 ) are observed as independent risk factors with the logistic regression analysis for the developing BSI?s. Hemodialysis ([exp (B) ]= 2,919 (CI= 1,469- 5,800)) and the time period of CVC application ([exp (B)= 0,867 ( CI=0,825- 0,911) are observed as independent risk factors for survival.Conclusion: There is an increase of the total cost, prolongation of hospital stay, and increase of the mortality rates are discovered at the patients who were exposed to nosocomial CVC related BSI?s. This results emphasizes taking the maximum precautions at the first application of CVC?s and the good regular catheter care is a essential to prevent nosocomial catheter related BSI?s.
