The role of multiparametric magnetic resonance imaging in differentiation between malignant and benign soft tissue and bone tumors

Abstract (EN)

Purpose The aim of our study is to evaluate the performance of multiparametric magnetic resonance imaging (Mp-MRI) quantitative data in distinguishing benign from malignant musculoskeletal tumors. Material and Method Thirtysix patients (18 benign, 18 malignant) who were histopathologically diagnosed as bone or soft tissue tumors and examined with Mp-MRI between July 2018 and December 2020 were included in the study. With the two methods determined, ADCmean and ADCmin parameters were obtained with measurements made on DK-MRI data by two observers at the workstation, Ktrans, Kep, Ve, Vp and iAUC data and DWI images, and the findings were evaluated retrospectively. In-class correlation coefficient was used to evaluate the agreement between the observers. The measured parameters were compared statistically between malignant and benign groups. The role of these measurements in determining the character of the tumor was evaluated statistically. Results All DK-MRI quantitative data (Ktrans, Kep, Ve, Vp and iAUC) were found to be significantly higher in malignant tumor tissue compared to normal tissue (p <0.05). Only iAUC value was found to be significantly higher in benign tumors compared to normal tissue (p <0.05). Ktrans and Kep values were found to be statistically significantly higher in malignant tumors than in benign tumors in the measurements made with standard ROI (p=0,011,p=0,013). Interobserver agreement was found to be excellent-good or moderate in the measurement of DC-MRI data. There was no statistically significant difference in ADC values between benign and malignant lesion groups (p> 0.05). Conclusion From the DCE-MRI quantitative data; Ktrans and Kep values performed with standard ROI were higher in malignant tumors than in benign tumors (P <0.05). It shows that Ktrans and Kep value, which is an indicator of increased permeability in the tumor, is an important parameter in determining the malignant character of the tumor.It is thought that the contribution of the method will become clearer with the standardization of measurement methods and technique in new studies
