Evaluation of patients who started peritoneal dialysis in the dialysis unit of muğla training and research hospital between 01.01.2012- 30.11.2022

Peritoneal dialysis (PD) is one of the safe and preserving methods of residual kidney function in patients who need renal replacement due to chronic kidney disease (CKD).Although the prevalence of PD in our country and in the world is low compared to other renal replacement therapies, recent studies have shown that hemodialysis provides a similar survival rate.In our study, it was aimed to investigate the demographic and clinical characteristics of patients who started PD, duration of peritoneal dialysis, the reasons for terminating peritoneal dialysis, complications related to peritoneal dialysis, peritonitis attack conditions and factors that may be associated with peritonitis attack. In our study, 117 patients who received PD treatment in Muğla Training and Research Hospital Internal Diseases Dialysis Unit between 01.01.2012-30.11.2022 were evaluated retrospectively.Age, gender, and comorbidities of the patients were recorded. The time and number of peritonitis attacks, peritonitis attack treatments, who opened the peritoneal catheter (by a nephrologist or surgeon), type and duration of PD, reasons for terminating PD, and control laboratory values that were checked during PD were recorded. The ages of the patients ranged from 6 to 89 years, with a mean age of 54,83±14,73 years.60,7% of the patients had hypertension, 36,7% had diabetes mellitus and 18% had congestive heart failure. It was observed that 10.3% of the patients were treated with APD and 72,6% with CAPD. The median value of the patients' stay in PD was 604 days, which was lower compared to the literature. It was determined that 48,7% of the patients switched to hemodialysis, 37,2% were ex and 9.0% had renal transplantation. Dialysis failure and PD related infections were found to be the most important causes of transition to hemodialysis. It was found that 44,4% of the patients had at least 1 episode of peritonitis. The most common microorganisms causing peritonitis were Staphylococcus aureus (9.4%), Staphylococcus epidermidis (5,2%) and Escherichia coli (3,1%). It was found that 38.5% of the patients developed PD related complications.The median creatinine value and parathormone value were found to be higher in patients who had a peritonitis attack compared to the patients who did not have a peritonitis attack (p=0,034).The median parathormone value was found to be borderline significantly higher in patients who had a peritonitis attack compared to the patients who did not have a peritonitis attack (p=0,050). As a result; in patients receiving PD dialysis; our findings in terms of demographic characteristics, comorbidities, PD method, PD outcome, causes of transition to hemodialysis, complications associated with PD, peritonitis and microorganisms causing peritonitis were found in parallel with literature data. More comprehensive studies with a prospective design and a large patient cohort are needed to confirm the results of our study.
