The mortality of deep hyponatremia in adult patients admitted to emergency service

Abstract (EN)
We are going to conduct this research retrospectively, after the approval of the ethics committee. Patients whose admitted to the emergency service of RTEU Eğitim ve Araştırma Hospital between the date of 01.01.2014 - 31.12.2015 and serum sodium level was lower than 125 mEq / L in emergency biochemical tests, 18 years old and older were included in this study. Patients who are pregnants or younger than 18 years old were not included in the study. Also hyperglycemic patients who had corrected sodium level of 125 mEq/L were not included too. Patients' age, gender, complaints of admission, additional diseases, discharge and admission status, duration of hospitalization, stituation of 1 month, 2 months, 3 months, 4 months and over then 4 months mortalities were examined. The statistical analyzes of the data obtained were compared with each other.221 patients were included in the study. 118 (53.4%) of the patients were female. The average age of patients was 75 (25-100) years. The mean serum sodium level was 121 (117-123) mEq / L. The most seen complaints were respiratory distress 61 (27.6%), nausea-vomiting 36 (16.3%) and loss of appetite 34 (15.4%). 137 (62%) of the patients had hypertension, 69 (31,2%) coronary artery disease, 63 (28,5%) congestive heart failure and 51 (23,1%) malignancy were the most common accompanying diseases. The most frequently consulted clinic was internal medicine 115 (52.0). The mean duration of hospitalization was 6 (3-12) days. Of the 221 patients, 102 (46.2%) were died. Mortality rate was higher in hospitalized patients. Of the patients, 117 (52.9%) had chronic hyponatremia. Mortality was more common in patients with chronic hyponatremia. There was no statistically significant difference in the mean serum sodium values of patients who died or not. There was not statistically significant difference in mean sodium levels between the mortality groups.Deep hyponatremia is more frequent in women and older. There is no difference in gender-related mortality. Because of the presence of accompanying diseases, we can't say that hyponatremia is the only responsible of mortality. It is not certain that the symptoms of the patients are secondary to the hyponatremia. Deep hyponatremia is an important cause of hospitalization and consultations to the other clinics. There is a significant relationship between deep hyponatremia and mortality. There is no significant relationship between mortality and mortality duration and serum sodium values in deep hyponatremic patients.
