The evaluation of attention deficit and hyperactivity disorder before and after adenoidectomy/adenotonsillectomy

Objective: The aim of this study is to determine the behavioral problems such as attention deficit, hyperactivity and impulsivity on patients that was planned for adenoidectomy or adenotonsillectomy, and investigate the effects of surgical treatment in that behavioral problems.Material and Method: This prospective study included 63 patients (29 female, 34 male) with obstructive symptoms that diagnosed adenoid or adenotonsillar hypertrophy based on clinical and history of previous, and was operated in Dicle University Faculty of Medicine, ENT service between January 2010 and May 2011. Age range was between 4-13 years old. Control group consisted of 33 (18 females, 17 males) healthy children. 20 questions questionnaire that was modified with DSM-IV diagnostic criteria for Attention Deficit and Hyperactivity Disorder get filled the parents of the patients in the preoperative period. Again, 20 questions questionnaire get filled to the parents of the patients in the 2-15. month of postoperative period.Results: Fifteen patients underwent adenoidectomy because of adenoid hypertrophy, 13 patients underwent adenotonsillectomy because of chronic adenotonsillar hypertrophy and the rest of patients underwent adenotonsillectomy because of chronic tonsillit or recurren tonsillit with adenoid hypertrophy. Patients? Attention deficit, hyperactivity and impulsivity data of preoperative and postoperative period was compared, and was found statistically significant difference (p = 0.000). Was found statistically significant difference between preoperative data of patient group and control group (p = 0.000). Was found statistically significant difference between postoperative data of patient group and control group (p <0.05).Conclusion: Behavioral problems such as attention deficit, hyperactivity and impulsivity is common in children with adenotonsillar hypertrophy due to signs of obstruction, and is observed decrease in these behavioral problems after adenotonsillectomy operation.
