Abstract (EN)
In this study, cases admitted within working hours, overtime, weekday, weekend admission of the differences in mortality aimed to be presented and searched ın Akdeniz University Medical Faculty Hospital, Department of Anesthesiology and Reanimation, Anesthesia Intensive Care II service between July 2009-July 2011. 1303 cases were included in the study. 59% were male cases (764 cases), 41% were women (539 cases) and their mean age was 55.6±20.2 years. According to the distribution of places of final acceptance of the cases examined, 755 patients (58%) were from operating room, 428 patients (33%) were from the emergency department and 120 patients (9%) were from the hospital's other services. 1120 (85%) patients were admitted during the weekday and 183 (14%) patients were accepted at the weekend in anesthesia intensive care unit. When you look athe mortality differences in admissions to intensive care unit during working inside and outside of business hours, the difference in mortality was significantly higher in admissions of outside of business hours. Mortality was 6.3% during the outside of working hours while the mortality was 3.2% in the overtime (p<0.05). When we investigated the mortality differences between weekdays admissions and weekend admissions, we identified that the difference in mortality was higher at the weekend admission (p<0.05). The mean APACHE II score was 14.6±5.8, SOFA score was 1.7±2.6. APACHE II scores were higher outside of working hours but no statistically significant difference was observed (p>0.05). APACHE II score of the patients were higher who were admitted to the ICU at the weekend. SOFA scores were significantly higher at night and weekend admissions (p<0.05). GKS scores of the patients who were admitted to the ICU at outside of business hours and weekend, were significantly lower. When we compare the duration of staying in the intensive care unit in terms of Cases taken out of working hours and in working hours, it presents that the cases which are taken out of working hours, stay longer in intensive care unit. Cases taken out of working hours stay 6.2±14.3 days, whereas the cases received in the working hours stay 4.0 ±9.9 days in intensive care unit.
