Comparing the effects of del nido and plegisol cardioplejia on myocard protection

Objective: Cardioplegic solutions have been developed to protect the ventricle during coronary artery bypass surgery. The effectiveness of each cardioplegic solution has been found to be different. To protect myocardium, Plegisol and del Nido cardioplegic solutions used in our hospital. The aim of this study is to compare the two solutions in terms of myocardial protection by interpreting the inotropic drug requirement of patients and their ventricular functions in echocardiographic imaging. Materials and Methods: Patients who underwent coronary artery bypass surgery with a cross clamp under cardiopulmonary bypass were divided into two groups (plegisol group (group 1 (n: 57)) and del Nido group (group 2 (n: 62)). The need for inotropic drugs and in the postoperative follow up, early ejection fraction and ventricular wall motion defects in the first echocardiography were evaluated. Results: The ratio of need for operating room inotropic drugs between the two groups; in the del Nido group was lower than the plegisol group (x²=8,323; p=0,004<0,05). Also, the rate of inotropic drug requirement was found lower at del Nido group during intensive care (x² =4,377; p=0,036<0,05). In the comparison of echocardiographic findings, the mean and changes of the ejection fraction, the rates of ventricular wall motion defects were similar in both groups (x² =0,213; p=0,644>0,05) and (F =0,314; p=0,516>0,05) Conclusion: In our study, the only advantage of del Nido cardioplegic solution to Plegisol cardioplegic solution is that it reduces the need for peroperative inotropic medication and as a result, it removes the heart faster than myocardial stunning. On the other hand, no significant difference was found in terms of other ventricular functions.
