Evaluation of placental localization and relationship with myometrium in previous cesarean sections

Abstract (EN)

The placental adhesion disorders whose incidence was relatively increased can cause several complications according to its types. Therefore, prenatal detection of the invasion and its type is very significant for the physician who will assist the birth. So far, placenta was the only tool used to detect these pathology but nowadays technological developments like doppler USG and MRI can be used. The aim of this prospective, descriptive, cross-sectional study is to detect placental adhesion disorders in recent cesarean sections with gray scale and doppler USG and to compare the results with histopathological diagnosis. The study was performed in Yıldırım Beyazıt University Medicine Faculty Ankara Atatürk Education and Research Hospital between January 2013 and July 2013. The patients were selected from 20-39 aged woman (mean age: 29,3) whose gestational week is 30,4 -39,6 (mean week 37,07) and who had at least one C/S in her priory births and plans C/S in coming birth. Totally 104 case were included in the study. İn order to detect adhesion disorders gray scale and doppler USG were performed in radiology department and then intraoperative bed biopsy were taken for histopathological investigations and the results were compared. Patients' name, protocol number, age, gestational week, height (cm), weight (kg), BMI, gravid, parity, number of living children, past history, family history, smoking status, operation except C/S and curettage, obstetric history (intrauterine ex, ectopic pregnancy, curettage, mol pregnancy, birth indications, placenta localizations, intraoperative placental adhesion defects, postoperative complications, gray scale and doppler USG findings, baby weight (gram) Apgar score in 5. minute, baby sex were all detected. None of the patients had placenta adhesion defects so that statistically no evaluation can be found between the findings of the patients with the placenta adhesion anomalies. The absence of placenta adhesion defects in our study can be explained with the surgery technique and factors like individual wound healing process. Although the risk factors of PYA are detected mechanisms concluding to abnormal placentation are not yet clear. In the aim of this larger studies are needed.
