The relationship between thiol / disulfide levels and oxidative stress and clinical parameters in patients with fibromyalgia syndrome.

Abstract (EN)
Fibromyalgia Syndrome (FMS) is defined as a chronic syndrome with diffuse body pain, fatigue, sleep disturbance, impaired cognitive functions and anxiety.There are many studies on the mechanisms of reactive oxygen species (ROS) that play a role in the etiology of FMS, but the role of oxidative stress in fibromyalgia is still controversial. Thiols, an important antioxidant, enter oxidation reactions with oxidant molecules and form disulfide bonds. The dynamic thiol / disulfide balance plays an important role in detoxification, antioxidant protection, signal transduction, apoptosis, regulation of enzymatic activity and transcription factors, and cellular signaling mechanisms. In this study, we aimed to investigate the relationship between thiol / disulfide levels and oxidative stress and clinical parameters that we think may play a role in the etiopathogenesis and clinical process of FMS. The study included 48 fibromyalgia patients and 38 healthy volunteers who met the 2013 ACR Fibromyalgia Diagnostic Criteria.In the evaluation of patients; WBC (White Cell Count), Sedimentation, CRP (C-Reactive Protein), VAS-Pain, VAS-Fatigue, FIQ (Fibromyalgia Impact Questionnaire), BDI (Beck Depression Inventory), BAI (Beck Anxiety Inventory), SF-36 ( Short Form-36), PSQI (Pittsburgh Sleep Quality Index), S-LANSS (Self-Leeds Neuropathic Symptoms and Signs Assessment), FSFI (Female Sexual Function Scale) were used.Serum NT (Total Thiol), TT (Total Thiol), TAS (Total Antioxidant Stress) and TOS (Total Oxidant Stress) levels were measured by enzyme-linked immunosorbent assay (ELISA).Disulfide (D); It was calculated as half of the difference between TT (Total Thiol) and NT (Native Thiol). OSI (Oxidative Stress Index) was calculated as TOS / TAS. NT / TT, D / NT and D / TT ratios were calculated from the results. There was no significant difference between the patients with FMS and the control group in terms of native thiol, total thiol, disulfide levels, native thiol / total thiol, disulfite / native thiol, disulfite / total thiol ratios, TAS, TOS and OSI levels.In our study, there was a negative correlation between NT and CRP, and disulfide and VAS-pain score. There was no correlation between NT, TT, disulfide levels, NT /TT, D/NT, D/TT ratios and TAS, TOS, OSI, WBC, sedimentation, number of tender points, duration of complaints, VAS-fatigue, FIQ, BDI, BAI, SF-36 total score, SF- v 36 sub-parameters, PSQI, S-LANSS and FSFI. In conclusion; We investigated the relationship between thiol / disulfide balance and oxidative stress and clinical parameters in patients with fibromyalgia syndrome. In our study, no statistically significant difference was found between the patient and control groups in terms of thiol / disulfide balance parameters, TAS, TOS, OSI values. There was a correlation between NT and CRP and disulfide and VAS-pain score. There was no correlation between other thiol / disulfide balance and clinical and laboratory parameters. In our study, we found that oxidative stress did not increase in patients with FMS. More extensive studies are needed to understand the role of thiol / disulfide and oxidative stress parameters in FMS etiopathogenesis and clinical process.
