Abstract (EN)
In case of premature rupture of membranes, the fetal membrane ruptures before the time for normal delivery. This can lead to potential complications in pregnancy and increase the risk of preterm birth. Premature rupture of membranes can often present with symptoms such as vaginal bleeding, fluid leakage, or pelvic pressure. However, since the diagnostic criteria for premature rupture of membranes have not yet been clearly developed, it is possible that the health of the mother and baby may face dangers. In this study, we investigate whether there is a relationship between fractalkine, fractalkine receptor, neutrophil elastase, sICAM-1, sVCAM-1, SLPI, IGFBP-1 and PAMG-1 concentrations in serum and amniotic fluid and EMR; It was aimed to evaluate the relationship between the parameters examined in order to obtain safer and more accurate results in EMR determination. The sub-objective of the research is to determine the concentrations of NE, sICAM-1, sVCAM-1 and SLPI in the amniotic fluid of women who had amniotic inflammation and/or infection in the second trimester and subsequently gave birth preterm or term. In line with the specified purpose, neutrophil elastase, sICAM-1, sVCAM-1, SLPI, Fractalkine and its receptor concentrations, and IGFBP-1 and PAMG-1 levels in serum and amniotic fluid were examined by the enzyme-linked immunosorbent assay (ELISA) method. Patient groups included in the study; 20 healthy pregnant patients without PROM who had a pregnancy between 37 weeks and 40 weeks and had a cesarean section due to elective reasons, and 22 pregnant patients with PROM who had a pregnancy between 24 weeks and 37 weeks and had a cesarean delivery. According to the research results in this study, a statistically significant difference was observed between the SLPI, neutrophil elastase, sICAM-1, sVCAM-1, Fractalkine and IGFBP-1 levels of pregnant women with PROM and healthy women in both serum and amnion concentrations, and while SLPI decreased in pregnant women with PROM, the other The levels of the parameters were found to be higher. In addition, there was a statistically significant difference in the PAMG-1 positivity of healthy pregnant women with PROM and the amnion control group and pregnant women with PROM, and it was higher in pregnant women with PROM; It was determined that this difference was not significant in serum. In light of the above results, the levels of SLPI, slCAM-1, sVCAM-1, neutrophil elastase, fractalkine, fractalkine receptor and IGFBP-1 in serum and amniotic fluid; It has also been determined that PAMG-1 levels in serum can be used as useful tests in evaluating the pathogenesis of PROM.
