Abstract (EN)

Objective: Patent ductus arteriosus (PDA) is a common condition in preterm infants that can lead to hemodynamic instability. This study aimed to retrospectively evaluate the hemodynamic changes in vital organs during pharmacological closure treatment of hemodynamically significant PDA using near-infrared spectroscopy (NIRS). Methods: This retrospective study analyzed 24 preterm infants diagnosed with hemodynamically significant PDA and treated in the Neonatal Intensive Care Unit of Selçuk University Faculty of Medicine between 2021 and 2024. The infants were divided into two groups based on their treatment: ibuprofen and paracetamol. NIRS measurements were performed to assess oxygenation changes in the brain, kidneys, and abdomen during the treatment process. Results: No statistically significant differences were found between the ibuprofen and paracetamol groups regarding gestational age, birth weight, and demographic characteristics. NIRS measurements revealed changes in cerebral, abdominal, and renal oxygenation levels throughout the treatment. Although some variations in oxygen saturation and organ perfusion parameters were observed between the two treatment groups, the differences were not statistically significant. Conclusion: NIRS can serve as a valuable tool in monitoring perfusion changes in vital organs during PDA treatment. The use of NIRS in preterm infants undergoing PDA closure therapy may help in better understanding treatment responses and contribute to individualized treatment strategies. Keywords: Patent ductus arteriosus, preterm infants, near-infrared spectroscopy, hemodynamics, ibuprofen, paracetamol, organ perfusion.
