Abstract (EN)

INVESTIGATION OF THE RELATIONSHIP BETWEEN THE DIETARY INFLAMMATORY INDEX AND SOME BLOOD BIOCHEMICAL PARAMETERS IN HEMODIALYSIS PATIENTS Chronic Kidney Failure (CRF) is a permanent and irreversible condition characterized by structural damage to the kidney. Medical nutrition is one of the most important supportive treatments in kidney failure. Foods can trigger unregulated immune system activation, leading to the onset of chronic, low-grade inflammation, which can impact proinflammatory and anti-inflammatory markers. The dietary inflammatory index (DII) is an index used to assess the anti-inflammatory or pro-inflammatory status of individuals' diets. The study aimed to evaluate the impact of the dietary inflammatory index (DII) on dietary patterns in individuals undergoing hemodialysis (HD) and to examine the relationship between the DII and biochemical markers associated with malnutrition and inflammation. Seventy-eight patients participated in the study. Patient demographics, dietary habits, anthropometric characteristics, malnutrition-inflammation scores, biochemical parameters, and a 3-day food record were used to assess the dietary inflammatory index. The average daily energy intake for female patients was 1020±279 kcal and for male patients was 1122±302 kcal. According to the Body Mass Index (BMI) values, 25% of the patients were slightly overweight and 23% were overweight. Waist circumference measurements indicated that 6% were at-risk and 55% were in the high-risk group. A statistically significant difference was found between the total MIS score and the DII quartiles (p<0.05). Furthermore, there was a positive, moderately significant correlation between the patients' total MIS score and the DII total score (r:0.361, p=0.001). A statistically significant negative correlation of "low strength" (r: -0.256, p=0.024) was found between DII total score and potassium (mmol/l), and a statistically significant negative correlation of "moderate strength" was found between DII total score and albumin (g/dl) (r=-0.565, p=0.001) and phosphorus (mg/dl) (r=-0.434, p=0.001). A statistically significant difference was found between the patients' NLR (Neutrophil/lymphocyte) values and DII quartiles (p=0.014). A statistically significant negative correlation of moderate strength was also found between the patients' DII total score and the nutritional parameters energy, carbohydrate, protein, polyunsaturated fatty acids, omega 3, dietary fiber, iron, potassium, phosphorus, zinc, magnesium, vitamin B6, niacin, vitamin B1, vitamin E, carotene, and folic acid. It is thought that an anti-inflammatory diet helps improve the course of the disease and reduces the risk of complications in hemodialysis patients.
