Investigation of the individualized care perceptions of patients and nurses with the professional self-concept in nurses

Abstract (EN)
Aim: In this study, it was aimed to examine the professional self-concept of nurses and individualized care perceptions of patients and nurses. Materials and Method: The sample of this research, which is planned and applied in a descriptive and cross-sectional pattern, is 100 nurses working in internal medicine, gynecology, orthopedics and traumatology, cardiology, neurology, urology and neurosurgery and general surgery services in a training and research hospital in Niğde and 16 August-30 November in these services. Composed of 357 patients who received inpatient and informed consent in 2019. In collecting data, Individualized Care Scale A / B, Indıvıdualized Care Scale A / B- Nurse, Professional Self-Concept Scale for Nurses and Clinical Nurses were used with personal information forms for patients and nurses prepared by the researcher. Data; In SPSS 22.0 program, using descriptive statistical analysis (number, percentage, mean, standard deviation, minimum-maximum value), chi-square analysis, correlation analysis, One-Way ANOVA test and t test in Independent Sample, 95% confidence interval and p<0,05. It was evaluated at the level of 0.05 significance. Ethics committee approval, Provincial Health Directorate permission and hospital approval were obtained. Results: The average age of the nurses participating in the study was 33.58 ± 7.05; It has been determined that 79% are women, 80% are married, 59% are graduates, and the Professional Self-Concept Scale for Clinical Nurses is 60.75 ± 12.44. A statistically significant difference was found between the mean scores of the Professional Self-Concept Scale for Clinical Nurses and some sub-dimensions according to some socio-demographic characteristics of nurses such as gender and income and expense status, and some professional characteristics such as working time in the service, job satisfaction level (p< 0.05). Nurses' Individualized Care Scale A-Nurse and Individualized Care Scale B-Nurse score averages were found to be 3.57 ± 0.73 and 3.67 ± 0.76. It was found that there was a statistically significant difference between the mean scores of Individualized Care Scale A-Nurse and some of its sub-dimensions according to some professional characteristics such as the way of working nurses, weekly working time, number of nurses working in the same code and job satisfaction (p<0.05). There was a statistically significant difference between the mean scores of Individualized Care Scale B-Nurse and some of its sub-dimensions according to some professional characteristics of nurses such as job satisfaction and number of patients who were cared for in a day (p<0.05). In the study, it was found that there is a positive weak relationship between the Nurses' Professional Self-Concept Scale for Clinical Nurses, Individualized Care Scale A-Nurse and Individualized Care Scale B-Nurse and some sub-dimensions (p<0.05, r = 0.467, r = 0.375). The average age of the patients participating in the study was 52.92 ± 14.37; 52.1% are women and 32.2% are primary school graduates; It was determined that the Individualized Care Scale A and Individualized Care Scale B score averages were 1.91 ± 0.66 and 2.05 ± 0.65. According to the socio-demographic characteristics of the patients such as gender, marital status, income and expense status, and social security type, the average scores of Individualized Care Scale A and some of its sub-dimensions are; According to some socio-demographic characteristics such as age, marital status, child availability and social security status, there was a statistically significant difference between the scores of Individualized Care Scale B and some of its sub-dimensions (p<0.05). According to some disease characteristics such as the reason of hospitalization of patients, the number of days of hospitalization, the number of nurses providing day care and knowing the name of the nursing nurse; It was determined that there was a statistically significant difference between the scores of Individualized Care Scale A and some of its subscales according to some hospital experience characteristics such as hospitalization and nursing satisfaction (p<0.05). According to some disease characteristics such as the number of days hospitalized by the patients, the number of days hospitalized in the service, the number of nurses providing day care and knowing the name of the nursing nurse; There was a statistically significant difference between the mean scores of Individualized Care Scale B and some of its sub-dimensions according to some hospital experience characteristics such as the presence of companion, the effect of life due to illness, some effects of the disease on life and satisfaction with nursing (p<0.05 ). Conclusions and Recommendations: The findings of the research suggest that the positive development of nurses' professional self-concepts may increase their perception of supporting individualized care and individualizing care; the perception that nurses support the individuality of patients and individualize the care of patients in their practice is higher than the perception that nursing actions that support the individuality of the patients are realized and their care is individualized; While the perceptions that support the individuality of the patient and individualization of the patient are high with the applications that take into account the individual characteristics of the patient individuals in the care provided by the nurses, the perceptions that the decision-making control is supported and the individualization of the care is individualized considering the personal life status characteristics in the care is high.
