Analysis of relationship between infantile colic and migraine and bioritm disorders

Abstract (EN)

Infantile colic is a common condition worldwide and effects 5-19% of babies under 3 month years old. Unstoppable crying attacks at least 3 hours in a day, at least 3 days in a weekend, and more than 3 weeks are characteristics of the disease. Crying attack occurs in the evenings in same periods in babies who had no other diagnosed diseases and ends by itself. Although, it is defined as a benign disease, in fact it is a stressful condition for parents. Unstoppable crying attacks are serious risk factor for shaked baby syndrome. A clinical study, which enrolled parents that have babies with infantile colic under 1 month old was showed that, 1% of parents was shaked and 2.2% of parents was slapped and also attempted to strangle their babies in order to stop crying. According to this study, these kind of harmful behaviors increase by the time and it becomes 5.6% in month sixth. Despite many trials, pathogenesis of the infantile colic is not clear. According to a theory, infantile colic may be related to delay in both neuro and gastrointestinal system development. In terms of another theory, anxiousness in family or inappropriate behaviors to baby can cause this disease. Old age and tobacco usage in mother and being a first baby maybe risk factors for infantile colic. Also some authors proposed risk factors such as intestinal microflora imbalance, upregulation of intestinal hormones ghrelin and motilin, hypersensitivity to cow milk and atopy can be related with infantile colic development. But nowadays, ethiopathogenesis of infantile colic remains still unclear. Case-control studies revealed that biorhythm regulation mechanisms continue to develop during postnatal first 3 months. Circadian rhythm of infantile colic attack, parallel progression in signs with age and disappearing symptoms after development of systems give rise to thought that pathogenesis of the disease can be related to biorhythm mechanism disorders. It is shown that, migraine history in parents is associated with migraine development in children. And on the other hand, lots of studies showed relationship between migraine and biorhythm regulation genes. Thus, in this study we aimed to asses whether migraine history have in parents of babies with infantile colic, and also analyses the relationship between infantile colic and biorhythm mechanisms. The study includes babies with colic pain administered to the Dokuz Eylül University Pediatric outpatient clinic and who has family consent. The control group was defined as babies without colic pain who administered to the same clinic and who has family consent also. In our study we planned to examine 46 infant as study group and 46 infant as control group in %95 confidence interval and with %80 of power. We applied a survey to the families both of study and control group. This survey examined the migraine history of parents with ID Migraine diagnostic scale which has been proven in terms of safety and efficacy in Turkey. We used SPSS 22.00 inc. statistic programme for analyzing the data of both group in %95 confidence interval and with %80 of power. As a result of our study, when the parents of colic infants were examined in terms of their behaviors during colic episodes, it was determined that the most frequent methods were flashing (78.3%), shaking (60.9%), car swinging (39.1%) and pacifier use (32.6%). Significant height of colic frequency was found in the infants whose mothers have university graduates (p = 0.002) and have professional occupation (p = 0.031). When the groups were compared in terms of sleep mode, the longest sleeping times in the colic group were significantly longer than the mean daily total sleep times (p = 0,828) and night waking frequencies (p = 0,339) in the colic group (p = 0,048). Comparing the infants' light and / or sound sensitivity, the sensitivity of light and / or sound was found to be statistically significantly higher (p = 0.001) in infants with colic symptoms. When the maternal headache and migraine status were assessed, the likelihood of colic in infants with migraine in their parents was statistically significantly higher (p = 0.002). Consequently new studies should be planned in relation to these findings and other studies in the literature regarding the relationship of infantile colic with biorhythm irregularities, clarification of the common pathogenesis and new treatment options.
