The effect of diagnosti̇c office H/S on pregnancy outcomes in patients with diagnosis unexplained infertility for intrauterin insemination

Abstract (EN)
Introduction and Aim: Infertility; It is an important problem affecting 10-15% of the general population. There are many different causes of infertility, but there is a group of 10-30% of patients whose cause of infertility cannot be found despite extensive evaluations. It is also emphasized that some patients with a diagnosis of unexplained infertility may have undiagnosed uterine pathologies, and that endometrial receptivity or tubal transmission may have decreased relatively. The aim of our study is to investigate the effect of office hysteroscopy at the beginning of the cycle on pregnancy outcomes in patients with unexplained infertility who will be treated with IUI. Materials and Methods: This study was carried out prospectively by including 91 patients who were admitted to the Department of Reproductive Endocrinology and Infertility at Bursa Uludag University Faculty of Medicine, Department of Obstetrics and Gynecology between January 2019 and April 2020, and who were diagnosed with unexplained infertility and planned for intra-uterine insemination. In 41 of the patients with undisclosed infertility, office H / S was applied at the beginning of the IUI cycle (study group), 50 patients started the cycle without the application of office H / S and were assigned as a control group. Both groups; biochemical pregnancy, implantation, clinical pregnancy and live birth rates were compared. Results: Biochemical pregnancy between the two groups (15.4%, 14% (p: 0.854), respectively) implantation rate (12.8%, 14% (p: 0.872), respectively) clinical pregnancy (12.8%, 14%, respectively) (p: 0.872)) and live birth (10.3%, 10% p: 1, respectively)). Only the BMI was found to be statistically significant in the study group, but it was not considered very reliable since the p value was calculated as 0.05. Conclusion: In asymptomatic infertile patients without transvaginal ultrasound findings, no positive effect of office H / S on pregnancy outcomes before IUI cycle was found. However, there are studies showing that pregnancy outcomes increase in patients with hysteroscopic pathology and correction.
