Relationship between blood pressure profile, arterial stiffness and osteoprotegerin

Introduction and Aim : Arterial stiffness is related with the increase of cardiovascular mortality and morbidity. Clinical studies showed us the relation between the arterial stiffness and age, sex, body- mess index, hypertension, diabetes and smoking. It is proved that, inflammation has an affect on arterial stiffness. OPG is defined as member of TNF receptor family. OPG increases the surviving of vein endothelial by blocking apoptosis. Even though the results aren't known clearly , the increase of the cardiovascular mortality and morbidity was seen due to the dipping pattern for the person who has non dipping pattern (blood pressure decrease less than %10 at nights). In our study, we aimed to find out the relation between the blood pressure profile (dipper, non-dipper hypertension) which is a risk factor and important point for defining long term prognosis in stiffness, the respond to the treatment of cardiovascular diseases which is foreseen the determinant measure of osteoprotegerin. Material and method : For this study, 36 patients -21 women and 15 men- applying Afyon Kocatepe University Medical School Department of Internal Diseases Policlinics between the dates June 2014 and June 2015, and who are diagnosed ,recently diagnosed with HT on ESC/ESH 2013 and JNC 8 recommendations according to hypertension diagnostic criteria and who are not on medication, and 34 healthy individuals-19 women and 14 men, as control group, who doesn't have systemic disease have been accepted. After their approval and routine evaluation, the patients and control groups have been integrated in the study. Findings: According to our findings we found out that the 24h average PWV (pulse wave velocity) has been determined as 7,56 ± 1,32 m/s in the hypertension group and 6,90 ± 1,71 m/s in the control group. From the stand point of 24h average PWV, a significant difference between these two groups. OPG levels have been noticed similar in the hypertension and control groups. It has seen that there is not a very clear difference as for the statistically, when we compared the patients' dipper and non-dipper with 24h average PWV. We have seen a positive relationship between the age and VKI increase with 24 h average PWV, with the correlation analyses and also the same with the systolic blood pressure and 24h average PWV. Results: At the end of this research, we prove that hypertension is a major risk factor on basis of arterial stiffness. In this context, we especially make it clear that higher the systolic blood pressure, there will be more arterial stiffness. In parallel with the other researches on this issue; we determine that there is a colleration between the arteriosclerosis and getting older. On the other hand, we think that there is no relation among OPG, HT and arterial stiffness. According to our point of view, this may originate from characteristics of experimental group and control group. In the light of our study, we consider that new studies should be developed between the OPG, arterial stiffness and blood-pressure profile.
