Comparison of partial nephrectomy results of patients with clinical stage T1a and T1b renal tumors

Aim: Comparison of partial nephrectomy results of patients with clinical stage T1a and T1b renal tumors Methods: A total of 142 patients who underwent open / laparoscopic / laparoscopic assisted open partial nephrectomy between 2010-2019 were evaluated retrospectively. Patients were divided into two groups as clinical stage T1a (tumor size 4 cm and smaller) and clinical stage T1b (tumor size greater than 4 cm and smaller than 7 cm). Both groups were compared in terms of demographic characteristics, operative and postoperative data, pathological results and follow-up data. Results: The mean age of the patients was 50,45 ± 14,10 and the mean body mass index was 26,53 ± 2,39. The mean age of the T1a group was significantly higher than the T1b age group. In the T1a group, the mass was mostly seen in the right kidney and in the T1b group, the mass was mostly seen in the left kidney. This was statistically significant. Renal score and mass size were significantly higher in T1b group. The mean duration of ischemia was 19,09 ± 5,52 in the T1a group and 18,39 ± 6,32 in the T1b group. No statistically significant difference was observed between the two groups. Although the duration of operation and bleeding were higher in T1b group, there was no statistically significant difference between the two groups. The rate of laparoscopic partial nephrectomy was found to be higher in T1a group. There was no significant difference between mean duration of hospital stay, duration of drainage and complication rates in both groups. There was no statistically significant difference in postoperative first month creatine elevations. Pathological subtypes were similar in both groups. The surgical margin positivity was %11,9 in the T1b group and %7,1 in the T1a group. Recurrence was observed in one patient in both groups. Conclusion: No statistically significant difference was found between surgical and oncologic results in both groups. Partial nephrectomy could be performed safely in tumors larger than 4 cm.
