Abstract (EN)
In last decades; VDR and 1α hydroxylase activity has discovered in many tissues and it's clear that Vitamin D (VD) has several roles in the organism, now. In this study we aimed to evaluate the association between serum 25(OH)D3 level and biochemical, inflammatory and nutritional parameters in chronic kidney disease (CKD) patients. We searched by using database of Akdeniz University Hospital and found 314 CKD patients as study group and 100 patients as control group who have met the criteria. We recorded age, gender, serum creatinine level, glomerular filtration rate (GFR), serum 25(OH)D3, calcium, albumin, phosphorus, parathyroid hormone (PTH) levels, spot urine protein/creatinine ratio, uric acid, neutrophile, lymphocyte, platelet counts, neutrophile/lymphocyte ratio (NLR), platelet/lymphocyte ratio (PLR) and mean platelet volume (MPV) for all patients and performed statistical analysis by using SPSS 23 statistical programme. We organized patients in three groups according to their VD levels. Groups consisted of patients with VD deficiency (VD level less than 10 ng/ml), VD insufficiency (VD level between 10-29 ng/ml) and normal VD levels (VD level 30 ng/ml and more). Participants grouped as stage 3 (GFR between 30-59), stage 4 (GFR between 15-29), stage 5 (GFR <15) according to classical CKD staging system. All groups were compared regarding their biochemical, inflammatory and nutritional status. No significant association was found between 25(OH)D3 and inflammatory parameters (CRP, NLR, PLR, MPV) in CKD patients while spot urine protein/creatinine ratio, albumine, phosphorus and PTH levels were significantly associated with serum 25(OH)D3 level. Especially spot urine protein/creatinine ratio and PTH level were higher in patients with VD deficiency. CRP, NLR, MPV, calcium, phosphorus, PTH, uric acid and PTH levels were also different among CKD subgroups and differences were statistically significant. In conclusion; there was no statistically significant association between 25(OH)D3 and inflammatory parameters in CKD patients while spot urine protein/creatinine ratio was significantly associated with 25(OH)D3 level in CKD patients. Serum 25(OH)D3 level and VD replacement may be important in management of proteinuria. We believe larger randomized controlled studies are needed at this point. Key Words: Inflammation, Vitamin D, Chronic Kidney Disease, Vitamin D and inflammation
