Abstract (EN)

49 7. SUMMARY Death associated with multiple injuries continues to pose a major public health concern, and ongoing efforts to reduce its impact remain paramount. A high percentage of patients (up to 30%) with multiple trauma encounter severe organ complications that lead to high morbidity in some cases. :.Tiaaa AraiiK 2002 ile Kasım 2003 arasında Günevdoeu Anadolu bölgesinde aamaan auşme, araç ıçı ve araç aışı trarîk kazası nedeniyle Dicle Üniversitesi Hastanesi Acil Servisinde tedavi edilen 1419 hastaya ait veri geriye dönük olarak analiz edildi ve ardışık olarak çalışmaya alındı. Motor vehicle accidents, falls from the roof of houses and homicide are the main causes of blunt multiple trauma. In our region, especially more intensively in the summer months, patients who suffer from blunt multiple trauma for various reasons ( falls from house roofs, injuries of passengers, pedestrians and drivers from traffic accidents) were admitted to Dicle University Emergency Service and prognosis of these patients seems to end with (lead to) mortality or morbidity. Several factors are effective on the mortality of these patients. Therefore, the purpose of our study was to research factors such as age, sex, admission time, the occurrence way of the accident (event) and serious anemic, head injury, chest injury, intraabdominal solid viscus injury presence at the admission and trauma scores (GKS, ISS, RTS) which, in our opinion, have effects on the mortality of the patients with blunt multiple trauma. In this study, 956 (%67.3) of the 1419 patients were men and the remaining 463 (%32.7) were women. And 111 of the 1419 patients who suffered multiple trauma became ex. Average age was 27,5+2,3 (1-92) and 20,6+0,5 (1-90) in the ex group and living group, respectively. When the occurrence time evaluated, it was noticed that the admissions of the cases mostly were in the summer months. By the usage of univariety statistical analyses, on the mortality we determine the significant effect of: being at 55 age or over (x2=23.67, PO.01), pedestrians and paserbys injured in traffic accidents (jj2=12,99, P<0,01), presence of serious anemi (X2=76.23, PO.01) at the time of admission, head (x*=319.46, PO.01) and thorax (X2=39.26, PO.01) damage, intraabdominal solid viscus injury (x2=93.80, PO.01), pelvis and/or femur fracture (j?=\ 1.72, P=0.01), we evaluated low GKS (t=48.27,50 P<0.01) and RTS (t=50.45, PO.01) scores by logistics regression (Backward Wald) method using multivariety analysis. Presence of the serious thorax damage (OR=0.51, CI=0.013±0.194, P<0,01), intraabdominal solid viscus injury (OR=0.184, CI=0.054+0.619, PO,05), being at 55 age or over (OR= 0.043, CI=0.013±0.144, P<0,01) low GKS score (OR=0.696, CI=0.574±0.844PO,01) and low RTS score (OR-0.337, CI-0.211±0.538, P<0,01) were found as predicted (effective) factors on the mortality of patients with blunt multiple trauma.
