Abstract (EN)
Objective: Comparison of wideband tympanometry results of Meniere's patients with those of individuals without Meniere's disease. Materials and Methods: 55 volunteers from Meniere cases who applied to Aydın Adnan Menderes University Hospital, Department of Ear Nose and Throat Diseases between December 2022 and May 2023 were included. Sociodemographic information, anamnesis, and drug use information that would affect the measurement result were evaluated with the Case Report Form of the participants. The audiological evaluation of the cases with normal otomicroscopic examination was evaluated in the OTATECH audiological double-walled acoustic cabinet with the Otometrics Madsen Astera II clinical audiometry device and noted on the case report form. Then, Interacoustic Titan broadband tympanometry test was performed on the subjects and frequency-specific absorbance values were measured. The Meniere's insufficiency scale scores of the cases were evaluated and recorded in the case report form (Appendix 3). In order to be compared with the case group, the frequency-specific absorbance values of 55 normal volunteers without Meniere's disease, whose otomicroscopic examinations were performed, were recorded on the Interacoustic Titan platform. The obtained data were analyzed in computer environment using SPSS (Statistical Package for the Social Sciences) program. Results: According to the answers given by the participants in the case group to the questions of Dokuz Eylül University Meniere's Insufficiency Scale, the relationship between attack period scores and sociodemographic characteristics was examined. A statistically significant difference was found between the attack period scores according to the marital status of the participants. The median value of the attack period score of the married participants is higher. A statistically significant difference was found between the attack period scores according to the labor force status of the participants. It was observed that the non-attack period score did not differ according to sociodemographic characteristics. It was observed that the total score did not differ according to sociodemographic characteristics.The patient in the case group was compared with the right ear GBT absorbance ambient pressure values with the control group. A statistically significant difference was found between the GBT absorbance ambient pressure values of the right patient ear and the right healthy ear at frequency values of 226, 500, 749, 1000, 1542, 3084, 4000 Hz. The median value of the case group is higher at frequency values of 226, 500, 749, 1000 Hz. The left ear GBT absorbance ambient pressure values of the patient in the case group were compared with the control group. A statistically significant difference was found between the GBT absorbance ambient pressure values of the left patient ear and the left healthy ear at frequency values of 226, 1542, 6168 Hz. The median value of the case group is higher at frequency values of 226, 500, 749, 1000, 1542, 2000, 3084 Hz. ROC analysis was performed by including the right and left ears of the patient and control groups for GBT absorbance values at frequencies of 226, 500, 749, 1000, 1542, 2000, 3084, 4000, 6168, 8000 Hz for ambient pressure. Significant results and area under the curve (AUC) were observed at 226 Hz and 500 Hz frequencies. The AUC value at a frequency of 226 Hz is 0.756. It was observed that using 0.095 as the cut-off point for 226 Hz frequency could detect Meniere's disease with 0.73 sensitivity and 0.34 1-selectivity. The AUC value at 500 Hz is 0.707. It has been observed that the use of 0.25 as the cut-off point for 500 Hz frequency can detect Meniere's disease with 0.73 sensitivity and 0.47 1-selectivity. The patient right ear GBT absorbance tympanometric peak pressure values in the case group were compared with the control group right ear values. Statistically significant difference was found in frequency values of 226, 500, 749, 1000, 1542, 4000 Hz. At frequencies of 226, 500, 749, 1000, 1542 Hz, the median value of the case group is high, and at the frequency of 4000 Hz, the median value of the case group is low. GBT absorbance tympanometric peak pressure values in the patient's left ear in the case group were compared with the left ear values in the control group. A statistically significant difference was found in the frequency value of 6168 Hz. At a frequency of 6168 Hz, the median value of the case group is low. ROC analysis was performed by including the right and left ears of the patient and control groups at frequencies of 226, 500, 749, 1000, 1542, 2000, 3084, 4000, 6168, 8000 Hz for GBT absorbance values for tympanometric peak pressure. Significant results and area under the curve (AUC) were observed at 226 Hz and 500 Hz frequencies. The AUC value at a frequency of 226 Hz is 0.717. It has been observed that using 0.085 as the cut-off point for 226 Hz frequency can detect Meniere's disease with 0.79 Hz sensitivity and 0.47 1-selectivity. The AUC value at 500 Hz is 0.680. It has been observed that the use of 0.25 as the cut-off point for 500 Hz frequency can detect Meniere's disease with 0.70 sensitivity and 0.47 1-selectivity. Conclusion: Among the sociodemographic characteristics we evaluated, we determined that only marital status was effective in the severity of the disease.Among the sociodemographic characteristics we evaluated, we determined that only marital status was effective in the severity of the disease. It was determined that the attack and non-attack period scores of married patients were higher. It was observed that symptom severity scales took different values according to the occupational status of Meniere's patients. While the symptoms are more severe in retired individuals, the symptoms are milder in worker patients.In Meniere's patients; Compared to the control group, higher frequency specific absorbance values at low frequencies and lower frequency specific absorbance values at high frequencies were observed. When wideband band tympanometry absorbance values of all frequencies were evaluated by ROC curve analysis, wideband band tympanometry absorbance values of 226 Hz and 500 Hz frequencies were observed to have significant sensitivity and selectivity.
