Relationship between metabolic syndrome acute phase reactants and 24 hour blood pressure monitoring

Abstract (EN)

Metabolic syndrome is an important predictor of type 2 diabetes and cardiovascular diseases that have high mortality and morbidity rate and has become a worldwide epidemic. We evaluated 98 (84 women, 14 men) patients who were attending Ankara Dıskapı Yıldırım Beyazıt Education and Research Hospital 2nd Internal Medicine outpatient clinic for metabolic syndrome using the criteria of the National Choesterol Education Program's Adult Treatment Panel III. Patients' acute phase reaction levels were studied; ambulatory blood pressures were monitored for 24 hours. Data were analyzed with SPSS 11.5 for Windows pack program. The most commonly observed component of metabolic syndrome was abdominal obesity (100%), followed by high blood glucose levels (93.9%), hypertension (83.7%) and in equal ratios low HDL (77.6%), and high triglyceride levels (77.6%). When classified according to their acute phase reactants, 55 (56.1%) patients have high CRP levels, 15 (15.3%) have high sedimentation levels, and 33 (33.7%) have high ferritin levels. After monitoring the patients for ambulatory blood pressure for 24 hours, it was found that 69.4% (68 patients) were in a non-dipper group while 30.6% (30 patients) were in a dipper group. We checked metabolic syndrome component distribution to see if there is a relationship between the acute phase reactant levels. Contrary to previous studies, no significant relationship was found between a metabolic syndrome criteria and acute phase reactants. According to the ambulatory blood pressure monitoring, individuals with metabolic syndromes have tendency to have higher ratio to be a non-dipper, however, when compared to the number of patients in the dipper group, the difference between the two is statistically insignificant. Cardiovascular complications, important cause of mortality and morbidity for individuals with metabolic syndrome, can be predicted using acute phase reactants and ABPM. With the help of acute phase reactant readings, possible development of a complication can be postponed using precautionary measures.
