Abstract (EN)

İntroduction: Intestinal microbiota is known to be a major contributor to lipid absorption, immunosystem defenses and energy metabolism. Although its exact metabolism is unknown, it has been shown that intestinal microbiota has a role in the development of obesity and it has been shown that the modification can increase energy production, induce low grade inflammation, increase insulin resistance and affect fatty acid tissue composition. Intestinal microbial alteration is thought to lead to intestinal inflammation leading to obesity. Several studies have shown that intestinal alkaline phosphatase plays a role in the regulation of intestinal microbiota. We aimed to investigate the relationship between intestinal alkaline phosphatase and obesity, which are important regulatory factors of intestinal microbiota in our study. Material and Method: 18 patients with BMI <25 kg / m2 , 18 patients with BMI between 26 and 30 kg / m2, and 18 patients with BMI> 30 kg / m2 ; who were referred to Ege University Medical Faculty Hospital Gastroenterology Endoscopy Unit between December 2017 and May 2018 ,were enrolled in the study. Those who met at least 3 of the NCEP ATP III criteria were diagnosed with Metabolic Syndrome and the patients were grouped according to Metabolic Syndrome. Serum and tissue samples from patients were studied with intestinal alkaline phosphatase enzyme ELISA. Western blot test was also performed to evaluate the presence of the enzyme in the tissue, and tissue and serum intestinal alkaline phosphatase levels were compared between the 3 groups according to BMI. Findings: 65.4% of the patients were woman and 34.6% of the patients were male. The mean age was 52.94 ± 14.93. The mean BMI was 29.36 ± 6.7 kg / m2 and the waist circumference was 101 ± 16.55 cm. The rate of smoking was 25%. Alcohol use rate was 11.5%. When patients were grouped as MeS, 34.6% received MeS diagnosis and 65.4% did not receive MeS diagnosis. The mean sIALP of the patients was 187.414 ± 76.532 ng / ml and the mean dIALP was 207,77 ± 122.68 ng / ml. In women, the mean level of the sIALP was 190.68 ± 82.30 ng / ml, while the mean of the sIALP in males was 66.041 ng / ml. There was no statistically significant difference between sex and mean sİALP. When the obese, overweight and weak group and the sIALP levels were compared it was observed that the highest average (x=247.910 ng/ml) was in the weak group, followed by overweight group (x=208.622 ng/ml) and obese group (x=112.433 ng/ml). When the obese, overweight and weak group and the dIALP levels were compared , again the highest mean (x = 306,638 ng / ml) was found to be in the weak group , followed by the overweight group (x = 232.236 ng / ml) and the obese group (x = 95.445 ng / ml). It was seen that the change with the sIALP and dIALP with BMI groups was statistically significant. When the DIALP levels are compared with the MeS diagnoses , the mean dIALP level was found to be 161.076 ± 106.532 ng / mL in the MeS group, while the mean DIAS level was 232.503 ± 124.87 ng / mL in the non-MeS group. Compared with the mean DIALP score between the two groups, the mean DIALP level in the MeS test group was statistically lower (p = 0.045). Conclusion: İn this study it has been shown that the components of İALP in the tissue and serum are lower in obese patients than overweight and normal overweight groups. In addition, it was determined that İALP tissue component was significantly lower in ca
