Abstract (EN)
The etiology of patients who underwent to the internal medicine outpatient clinic with the complaint of fatigue is hypothyroidism. TSH level is routinely detect from patients determining hypothyroidism. If the patient's TSH level is found to be above normal and the control TSH level performed on the same day is found to be high, the patient is prediagnosed as hypothyroid and advanced tests are started in terms of hypothyroidism etiology. However, it can generally be overlooked that the reason for the patient's TSH elevation may be improper sleep habits for the circadian rhythm. In this study, we aimed to evaluate the change of TSH level in the blood of individuals who did not sleep in accordance with the circadian rhythm and the changes between the states of the same individuals who slept in accordance with the circadian rhythm. Our study focused on the necessity of sleep status in the evaluation of TSH level. 100 patients who underwent our study with the complaint of fatigue, whose TSH was found to be above the normal limit in the examinations and who had a history of insomnia were included in our study. In our study, the TSH level in sleepless men and women was compared with the status of sleeping in accordance with the circadian rhythm of the same men and women. Our results were found to be significant. When the results of our study were analyzed, it was observed that the TSH level was higher in sleepless women than in the same women who slept in accordance with the circadian rhythm. Similarly, it was observed that the TSH level of sleep-deprived men was higher than that of the same men who slept in accordance with the circadian rhythm. In the light of the evaluations, TSH elevation should detected in patients who underwent with the complaint of fatigue, it should first be interrogated whether they sleep in accordance with the circadian rhythm. When the person's sleep status is found to be inappropriate to the circadian rhythm, the TSH analysis should be repeated after the person sleeps in accordance with the circadian rhythm. In this way, unnecessary further tests will be prevented, and it will be ensured that the wrong diagnosis of hypothyroidism is prevented.
