Startle epilepsi ve juvenil myoklonik epilepsi hastalarinda transkranial magnetik stimulasyon kullanilarak motor eşik değerlerinin ve kortikal eksitabilite farkliliklarin araştirilmasi

Materials and Methods: 4 patients diagnosed with startle epilepsy(3 male, 1 female), 9 patientsdiagnosed with JME (6 male, 3 female) and 10 healthy volunteers (9 male, 1 female) wereincluded in this study. All participants were informed about the study and their written approvalswere taken. Using transcranial magnetic stimulation (TMS) motor threshold (MT) values,through SICI (short intracortical inhibition), LICI (long intracortical inhibition) and ICF(intracortical facilitation) valuation their differences were researched.Results: The average Motor Threshold (MT) values of control group were found statisticallysignificantly lower than those of the Startle and JME groups (p=0,004, p=0,002), there was nostatistically significant difference observed between Startle and JME groups (p=0,163). Amongthe stimulation intensity averages of the Startle, JME and Control groups, there was statisticallysignificant difference observed (p=0,0001),the stimulation intensity average of the Control groupwas found statistically significantly lower than those of the Startle and JME groups(p=0,004,p=0,002), the stimulation intensity averages of the Startle group was found statisticallysignificantly higher than that of the JME group (p=0,005). Among the groups, SICI amplitude,ICF amplitude difference was not observed.Discussion: With the help of the literature on the subject, higher MT values in startle epilepsyand JME patients were thought to be related to the anti-epileptic drug therapies. SS in startleepilepsy patients was observed to be higher than that in the JME and control groups; and this wasthought to be related to the multiple anti-epileptic drug therapies applied to startle epilepsypatients. While we cannot make definite conclusions, as there are different results in theliterature, a broader study needs to be conducted by increasing the number of the patients used inthe study
