Abstract (EN)
The goal of the current study is to identify the efflux pump found in Escherichia coli isolated from urinary tract infections, as well as its association to antibiotic resistance. A total of 100 clinical samples were taken from the urine of pregnant women with urinary tract ınfections (UTIs) from various hospitals in Thi-QAR. Traditional methods were utilized to identify bacterial isolates, including antibiotic sensetivity test their and PCR to detect the 16S rRNA and gene coding for bacerial resistance to antibiotics. Total collected samples number was 100 of which only 49 (100%) were positive for bacterial isolates, including 22 (44.9%) E. coli, 11 (22.4%) Klebsiella pneumonia, 3 (6.1%) Klebsiella oxytoca, 6 (12.2%) Proteus spp., 5 (10.2%) Enterobacter spp., and 2 (4.1%) Pseudomonas aeruginosa. Beta lactam antibiotics resistance E. coli and Klebsiella spp. were tested and outcomes were 19 (86.4%) from 22 for E. coli and 13 (92.8%) from 14 for Klebsiella spp. this number represent 32 (88.8%) of all tested 36 samples. ESBL production in E. coli and K. pneumonia results revealed that only 4 (12.5%) from 19 samples were positive and 5 (15.6%) from 13 were positive in K. pneumonia represented 9 (28.1%) from 32 isolates. Genes of bla TEM and bla SHV genes in ESBL positive E. coli and Klebsiella spp. isolates 2 (50%) of 2 tested E. coli isolates were positive for both bla TEM and bla SHV genes, while 3 (60%) and 2 (40%) of Klebsiella spp. isolates were positive for the same genes, recording 5 (55,6%) and 4 (44.4%) for the two genes respectively. Regarding bacerial resistance to antibiotics isolates tested positive for MDR for E. coli anlow for amikacin, piperacillin and ceftriaxone, while it was high for gentamycin, ceftazidime, nalidixic acid, cefixime, ciprofloxacillin, cefotaxime and cefipime respectively. This study reveales that there is a need to apply urine culture and sensitivity test to assess urinary tract infection (UTI) among pregnant women and detection of gram-negative resistant bacteria, this will help the patients to get safe and effective treatment. There is high prevalence of multi-drug resistance genes and in practical routine daily use among E. coli and K. pneumonia.
