Abstract (EN)
This research was conducted to determine the opinions of nurse managers on nursing care delivery methods and to evaluate the effects of primary nursing method on nurse and patient outcomes. The first phase of the research employed descriptive design using phenomenology method from qualitative research methods, and the second phase using semi-experimental research method, which is one of the quantitative research methods, with control group pre-test post-test in non-randomized groups. The sample of the first phase consisted of a total of 28 nurse managers as eight seniors and 20 juniors, working in the Ministry of Health hospital and private hospitals. The sample of the second phase consisted of a total of 63 nurses, including 31 in experimental group and 32 in control group, working in the general surgery, internal medicine and orthopedics units of an educational and research hospital in Istanbul, and a total of 1243 patients, including 669 in experimental group and 574 in control group, who were hospitalized during the start of the study, and on the third and sixth months. The power of the study was calculated using the "G. Power-3.1.9.2" program. The results of the power analyses were at an acceptable level, while an adequate number of data was reached. The "Semi-Structured Interview Form" was used in the interviews conducted in the first phase of the study. The Nurse Identification Information Form, MISSCARE Survey, Nurses' Role and Competencies Scale (NRCS) and Job Satisfaction Scale for Clinical Nurses (JSS-CN) for nurses, and the Patient Identification Form, Newcastle Satisfaction with Nursing Care Scale (NSNS), MISSCARE Survey-Patient for patients were used in the second phase. Main and sub-themes were created by using the MAXQDA 2022 program from the data obtained in the first phase of the research. In the second phase, the data were analyzed using the Statistical Package for Social Sciences (SPSS) for Windows 25.0 package program. The data were analyzed using descriptive statistical methods (number, percentage, mean, standard deviation, minimum, maximum) and nonparametric tests. The Mann Whitney U test was used for the difference between the two independent groups in the comparison of quantitative data without a normal distribution. As a result of the content analysis of the data obtained from in-depth individual interviews in the first phase, four main themes were obtained as "Creation of study schedules", "Nurse assignment criteria", "Reasons for choosing the applied assignment method" and "Results of the applied assignment method" and a total of 23 sub-themes under these themes. In the second phase, the second-test MISSCARE Survey scores of the control group nurses were found to be higher than those of the experimental group nurses and the difference was statistically significant, while the second-test MISSCARE Survey scores of the experimental group nurses were lower compared to the pre-test scores, with a statistically significant difference. No statistically significant difference was found between the intragroup pre-test, first-test and second-test JSS-CN scores of experimental and control nurses. No statistically significant difference was found between the intergroup pre-test, first-test and second-test NRCS scores. No statistically significant difference was found between the intergroup pre-test, first-test and second-test NSNS scores of care-provider nurses. The patients receiving care from control group nurses obtained higher MISSCARE Survey-Patient pre-test and first-test scores compared to those receiving care from experimental group nurses, which was statistically significant, and in addition, they obtained higher MISSCARE Survey-Patient communication subscale first-test scores compared to those receiving care from experimental group nurses, which was also statistically significant. The pre-test, first-test and second-test MISSCARE Survey-Patient timing subscale scores of patients receiving care from control group nurses were found to be higher compared to those receiving care from experimental group nurses, and the difference was statistically significant. It was revealed that the primary nursing method has an effect on missed patient care. In future studies, it is recommended to evaluate the effects of different care delivery methods on patient, nurse and institutional outcomes.
