Treatment results of pediatric patients with non hodgkin lymphoma receiving treatment between 2012-2020

Abstract (EN)
Objective: In our study, it was aimed to evaluate the prognosis and response to treatment protocols of pediatric patients diagnosed with Non Hodgkin Lymphoma in our center between 2012 and 2020, taking into account their demographic and clinical characteristics. Materials and Methods: The data of the patients were analyzed retrospectively from patient files and general hospital data system. From patient files; age, gender, histopathological diagnosis, first and last admission dates of the patient, recurrence and survival, duration of follow-up, stage of the disease, location of the mass, bone marrow involvement status, treatment protocol, complete blood count and biochemistry values at the time of diagnosis, pleural effusion, ascites, lymphadenopathy and Presence of hepatomegaly, radiotherapy status, family history of cancer, chemotherapy complication development, leukemic transformation, EBV positivity, CD20 positivity, bone marrow transplantation status were examined. Patients who left the center before their treatment was completed and patients who started treatment in another center and continued in our hospital were not included in the study. Results: 90 patients were included in the study. It was determined that 75.6% survived, 21.1% died, and 3.3% were out of follow-up. 66.7% of the patients were male. 64.4% of the patients were under the age of 10. When histological subtypes are examined; 31.1% were LL, 47.8% were BL. It was determined that the incidence of advanced disease (stages III and IV) was higher. Recurrence was seen in 16.7% of the patients. Recurrence was detected in 10% of patients with stage I-II-III and in 34.6% of patients with stage IV. A statistically significant correlation was found between being at an advanced stage and the development of recurrence. There was a statistically significant difference between bone marrow involvement and/or LDH value 2 times or more above normal and advanced stage. It was determined that the 3-5-10 year overall survival of 15 patients who developed relapse was 68%-52%-52%, respectively. It was determined that the 3-5-10 year overall survival of the patients who did not relapse was 83% on average. This situation was found to be statistically significant (p=0.017). Conclusion: An increase in event-free survival is observed with the determination of the risk groups of the patients and the application of appropriate chemotherapy protocols. NHLs are rapidly progressive malignancies with an aggressive course. Therefore, delay in diagnosis or low patient compliance leads to progression of the disease. Physicians should be more informed about the signs and symptoms of childhood cancers and their awareness should be increased.
