Abstract (EN)

Objective: This study was conducted to compare the analgesic efficacy and soft tissue healing following treatments with alvogyl, photobiomodulation, A-PRF, and pentoxifylline in patients with alveolitis. Method: The study was conducted on 80 patients who presented to our clinic with complaints of post-tooth extraction pain, aged between 18 and 65 years, and without any systemic diseases. The patients were divided into four groups as follows: Alveogyl (n=20), A-PRF (n=20), Photobiomodulation (n=20), and Pentoxifylline (n=20). In patients diagnosed with alveolitis, the control group was subjected to irrigation with saline solution followed by the application of alveogyl. For the A-PRF group, after irrigation with saline solution, Advanced Platelet-Rich Fibrin (A-PRF) obtained by centrifuging 10 ml of blood from the patient in a plastic tube without anticoagulant at 1500 rpm for 14 minutes was placed into the extraction socket. For the photobiomodulation group, following irrigation with saline solution, intraoral photobiomodulation was performed using a 810 nm wavelength diode laser (Picasso Dental Diode Laser, company AMD lasers, USA) with a deep tissue tip at 20-25 J/cm² and 1.0 W per cm², set with a spot size of 300 nm diameter (non-contact and continuous mode, 20 seconds) on occlusal and buccal areas (on postoperative days 0, 2, 4, and 7). For the Pentoxifylline group, following irrigation with saline solution, Trental retard 400 mg tablets were prescribed and administered 2 times a day for 1 week. Postoperatively, patients' pain levels were assessed by asking the patients using the Visual Analog Scale on days 0, 2, 4, and 7, and scored on a scale of 0-10 (0: no pain, 10: unbearable pain). On day 0, postoperative 1st and 2nd weeks, the Landry Healing Index was evaluated. Granulation tissue measurements were taken from patients on days 0, 2, 4, and 7 postoperatively. Granulation tissue measurement was performed by measuring the tissues on the walls of the alveolar socket. Following the data collection, the findings were statistically evaluated. Results: Similar results were found among the groups in terms of pain. In terms of soft tissue healing, alveogyl and photobiomodulation showed statistically significant superiority (p=0.001) compared to the other groups. Conclusion: The application of alveogyl and photobiomodulation for promoting soft tissue healing and pain control in cases of alveolitis yields better clinical results compared to pentoxifylline and A-PRF, in terms of clinical findings.
