The use of doppler ultrasound ın evaluatıon and follow up of A-V fıstula patıents

103 patients who has applied for A-V fistula operation was assesed through march 2010 ? october 2010 by 2010/3 numbered permission of Celal Bayar University ethical committee. Exculusion criteria were; previuos application of hemodialysis catheter, history of upper extremitee thrombophlebitis, history of upper extremetee surgery and history of hospitalization during the last two months . Among the remaining patients, ones who meet the criteria of color Doppler US and physical examination are included in study.In order to eliminate randomization obligation of patient group, for all the patients brecio-cimino method was chosen. Patients with chronic renal failure and planned A-V fistula formation for the first time with the purpose of hemodialysis, were divided into two groups; Control group (20 patients) and Case group (20 patients) by sequential randomization. For 20 cases of control group, arterial and venous system evaluation was done by physical examination. For 20 cases of Case group that surgeon was blinded, Brecio-Cimino operation was applied in the extremities that the ultrasonographic evaluation recommended after arterial and venous color Doppler US was done for both of the upper extremities before the operation.Both groups were examined for A-V fistula function, thrill palpation and use in hemodialysis in 10th day, 3rd and 6th months and 1st year after the operation. All gathered information was statisticaly analysed.As a conclusion, A-V fistula formation operation is a special operation that can be managed in experienced centers in hands of experienced surgens for selected patients with high care. Doppler US evaluation and selection of most proper vessels prior to A-V fistula surgery, significantly decreases the number of unsuccesfull operations.
