STUDY OBJECTIVEThe aim of study the was retrospective assessment of preferred treatment modalitiesand their efficacy of rhythm control in patients admitted to our emergency department (ED)with acute atrial fibrilation.MATERIAL AND METHODAdults admitted to our ED between 01.01.2001-01.11.2009 and patients diagnosed ofacute AF in the first 48 hours and without exclusion criteria were included in the study.Patients?s chart records were reviewed retrospectively.RESULTS380 AF patients with acute onset and hemodynamically stable were included in thestudy. The mean age of patients was 61,9 ± 0,7 and 220 (57,9 %) of them were women.In our ED in the 79,5 % (302) of the patients with acute AF, drugs were applied toreduce ventricular rate. When the results rhythm of the patients were examined, 308 (81 %)patients returned to sinus rhythm. It was identified that in the ED follow up 95% of thepatients returning SR were in the first 12 hours of observation, 99,4 % of the patients were inthe first 24 hours. The average duration of patients converted SR was measured as 4,6 ± 0,3hours (range 0,2 to 29 ). In our ED, in 74 % of 38 patients followed up without any treatmentwere provided SR spontaneously during observation. In the 82 % of 182 patientsadministrated only the speed control, in 87,5 % of 40 patients used only rhythm control, in 80% of 120 patients used both the rate and rhythm control, SR was achieved.To return SR rhythm; compare with patients who received speed control medication inlast 24 hours (113) and not received group (267), there was no significant differences duringED observation and medical treatment (P>0.05). Medical treatment and observation when theSR turns of the patients use and not use in the last 24 hours speed control medication werecompared, there wasn?t superiority between the sub-groups. Compare for return to SR whouse rhythm control drug before presented emergency department in the last 24 hours,propafenon were significantly effective than amiodarone (P=0,001).4The majority of patients (70 %) were discharged after ED observation, only 30 % ofadmitted the cardiology or intensive care unit. 47 % of hospitalized patients provided SR inED.CONCLUSIONIn our study, in the result of treatment in the ED the large parts of the patients withacute onset AF returned to SR and for most of them the admission wasn?t necessary.When the treatment methods applied in the ED (drugs not applied, the speed controlapplied, the rhythm control applied, the speed and rhythm control applied) compared with SRturnsof patients there wasn?t any significant advantage among sub-groups (P=0,736).KEY WORDSAtrial fibrilation, treatment, rhythm control
