Diagnostic value of plasma spexin in patients diagnosed acute coronary syndrome in emergency department

It was aimed to investigate serum specin levels in patients diagnosed with myocardial infarction (MI) in the emergency department. A total of 200 patients, including 50 patients with ST elevation myocardial infarction (STEMI) and 50 patients with non-ST elevation myocardial infarction (NSTEMI), 50 patients with atypical chest pain and 50 healthy and volunteer control groups, who applied to the emergency department with chest pain complaints. The mean spexin level (562.78±325.87 pg/ml) of STEMI patients participating in our study was compared to atypical chest pain (821.60±513.87 pg/ml) and control groups (812.10±476.17 pg/ml) was significantly lower(p<0.001, p<0.001, respectively). Similarly, the mean spexin level (574.38±393.24 pg/ml) of NSTEMI patients was significantly lower than that of atypical chest pain and control groups (p<0.001, p<0.001, respectively). There was no significant difference between the spexin levels of the patients diagnosed with STEMI and NSTEMI (p=0.829). In the ROC Curve analysis, when we take the cut-off value of spexin as ≤532 pg/mL in the diagnosis of STEMI, its sensitivity is 60%, its specificity is 76%, its positive predictive value (PPV) is 63.7%, and its negative predictive value (NPV) is 63.9% (AUC=0.703 p<0.001, 95% CI: 0.603-0.790). In the diagnosis of NSTEMI, when we take the cut-off value of spexin as ≤520 pg/mL, the sensitivity is 56%, the specificity is 78%, PPV is 71.8%, NPV is 63.9% (AUC=0.704, p<0.001, 95% CI: 0.604 - 0.791). There was no significant correlation between Spexin level and HEART and TIMI scores in MI patients. As a result; Spexin levels are significantly decreased in STEMI and NSTEMI patients.
