The relationship of clinical and laboratory parameters with computerized tomography findings of pulmonary angiography for diagnosis of pulmonary emboly in emergency department

Abstract (EN)
The aim of this study is to determine the 'clinico-radiological markers' in determining the diagnosis and mortality of PE by examining the clinical findings with CTPA images taken for the diagnosis of PE in the emergency department. For this purpose, PE cases were analyzed by designing as a retrospective cohort study. The patient population is patients over the age of 18, who applied to the Emergency Department of Akdeniz University Medical Faculty Hospital between January 2019 and July 2021, and were diagnosed with pulmonary embolism by computed tomography pulmonary angiography. 268 patients were included in the study, but patients whose CT parameters could not be measured due to contrast mismatch, and 41 patients whose treatment or outcome criteria could not be determined were excluded. 227 patients were included in the analysis. The mean age of PE patients admitted to the emergency department was 59.73 years. Sixteen (7%) of the patients included in the study had a history of pulmonary embolism. 16 (7%) had a history of recent surgery, 5 (2.2%) had a history of heart failure, 36 (15.9%) had a history of chronic lung disease, 83 (36.6%) had a history of active has a history of malignancy. 70 of them (30.8%) consist of patients over 70 years of age. 87 (38.3%) had chest pain, 155 (68.3%) had shortness of breath, 17 (7.5%) had palpitations. Twenty-four (10.6%) of the patients included in the study died during primary follow-up. In conclusion, clinico-radiological factors determining mortality in patients with PE; History of PE, chest pain and respiratory rate above 30/min, aortic diameter and pulmonary artery diameter greater than normal, presence of pulmonary infarction and higher than normal Qanadli score were found.
