Reliable markers additional to biomarkers establish the prognosis would be highly beneficial in heart failure.We sought to determine influence of accompanying pleural effusion on in-hospital outcome and prognosis at 6-month follow-up in acute decompansated heart failure (ADHF) and relation with levels of CA 125 and NT-pro BNP.Study consisted of 100 consecutive patients. CA 125 and NT-proBNP were measured. Patients were divided according to presence of pleural effusion. In-hospital outcome and 6-month rehospitalization and mortality were recorded. There were 27 total deaths over the course of 6-month follow-up. Insignificant trend towards higher values of CA125 was found in patients with pleural effusion. Univariate Cox regression model showed that there is no influence of pleural effusion on in-hospital outcome and mortaity at 6-month follow-up (p=0.4). CA 125 predicted mortality by 1.29 (1.13-1.47), (p<0.001); NT -proBNP predicted mortality by 1.08 (1.03-1.14), (p=0.004). Multivariate Cox regression analysis showed that only CA 125 was independent predictor of prognosis at 6-month follow-up [RR: 1.2 (1.04-1.4); p=0.001].In patients with ADHF, there was no influence of accompanying pleural effusion on mortality and rehospitalization at 6-month follow-up. CA 125 found to be an independent indicator of prognosis irrespective of pleural effusion.Key words: Decompansated heart failure, pleural effusion; CA 125; NT- proBNP, prognosis.
