Abstract (EN)

Preoperative And Postoperative Psychiatric Assessments Over the Operated Patients Due to Lung Cancer Objective: Operated patients due to lung cancer were intended to be explored in terms of their preoperative and postoperative psychiatric situations. Materials and Methods: Over the 25 patients operated in our clinic with lung cancer diagnose in prospective actualized study upon the assessment of the patient's psychiatric conditions; in preoperative period Life Inventory in Close Relations II; EORTC QLO C-30 Life Quality Scale; Perceived Family Support; Stress Thermometer and Hospital Anxiety Depression Scale in combination; postoperative at 1st month Perceived Family Support; Stress Thermometer and EORTC QLO-30 Life Quality scales were implemented. Results: 17 out of patients (68%) were male, 8 of them (32%) were female and age average corresponded to 61±8.9 (38-81). Patients diagnosed under 44% in terms of depression, in terms of anxiety found under 28%. With respect to anxiety between genders while no statistical significance was found (p=0.088); over the women higher depression ratios were diagnosed compare to males (p=0.03). Among the candidates with intense preoperative stress magnitude on the life quality evaluation scale of EORTC QLQ C-30; emotional functions related life quality sub-dimension (p=0.09) and life quality social functions correlated sub-dimension found deregulated (p=0.17) insomnia (p=0.012) and appetite loss found elevating (p=0.038). In association with preoperative anxiety level and EORTC QLQ C-30 life quality scale when compared; over the patients with no anxiety in comparison to the others with anxiety life quality overall functionality (p=0.019) and emotional functionality scores (p=0.015) were found higher, in due course in these segments life qualities attenuated; due to lower diarrhea scores (p=0.024) in these fields life qualities progressed; over the anxiety patients dyspnea (p=0.043) and insomnia (p=0.037) scores elevated, so that, in that scope life qualities found deregulated. 5 out of 18 patients with negligible anxiety score (27.8%); in 3 out of 7 patients with high score (42.9%) postoperative complications were monitored. 3 out of patients with negligible depression score (21.4 %); in 5 out of 11 patients with high score (45.5%) postoperative complications were monitored. In 40% of the preoperative low stress level patients; in 30% of the patients with intensified stress postoperative complications were found. Over the ones with no preoperative malignity diagnose anxiety score were higher than the malignity diagnosis. Over the entire female patients preoperative stress level while found intensified over the 70.6% of the male patients that level was elevated. Preoperative applied scales when compared with gender while preoperative stress level (p=0.045) depression presence (p=0.030) and insomnia (p=0.033) found over women higher than male; life quality sensitive (p=0.003) and social (p=0.000) functional segments over the male found worse than women. Upon the execution of general evaluation, operation was found as attenuating life quality (p=0.046) and elevating exhaustion (p=0.036). Conclusion: Findings set forth crucial clues in terms of patient's experienced psychosocial depressions and adaptation to the therapy. Due to lung cancer patients selected for operation in postoperative period must be taken into assessment based on hospital anxiety-depression scale and stress thermometer. Of the patients having higher stress level and under risk due to anxiety and depression preoperative psychiatry consultation must be claimed. Psychosocial support and treatment support to the cases is conceptualized by us considering the fact that shall be constructive in terms of life quality progresses and deregulate postoperative complications. In that subject more detailed multi-central studies incorporations shall lead to better understanding of psychiatric destructions and relevant concerns over the lung cancer patients.
