Abstract (EN)

Introduction and Aim: The complaint of dyspnea is one of the common symptoms among emergency department admissions. Therefore, it has a wide diagnostic range in terms of differential diagnoses. BNP is one of the tests used both in the diagnosis of heart failure, which is one of these diagnoses, and in the evaluation of the response to treatment. The primary aim of our study is to reveal the relationship between BNP and 1-month mortality. The secondary aims of our study are to determine the relationship between 1-month mortality and Troponin, Age, Gender, Hypoxia, Hypotension, Thorax Computed Tomography. Materials and Methods: This study was carried out with patients who applied to the adult emergency department of Akdeniz University Hospital between 11 March 2020 and 09 April 2021 and whose BNP levels were studied. The 1-month mortality data of the patients were obtained by retrospectively examining the hospital information management system and patient files, and additionally scanning the Death Notification System. Patients under the age of 18 or pregnant were not included in the study. Patients with missing BNP data and mortality data were excluded from the study. Results: Of the 1232 patients included in the study, 539 (43.8%) were female and 693 (56.2%) were male. The minimum age of the patients included in the study was 19, the maximum age was 100, and the mean age was 68.9±13.14 years. The number of patients who died within one monthfrom 1232 patients who applied was 171 (14%). When the factors affecting the mortality of the patients included in the study were examined, BNP level (p=0,004), troponin level (p<0,000), age (p=0,001), hospitalization status (p<0,000), hypotension (p<0,000) and hypoxia (p= 0,003) was found to be statistically significant. No statistically significant relationship was found between the death of the patients and gender (P=0,763). No significant correlation was found between the results of heart failure in Thorax Computed Tomography and mortality (p=0,328). Conclusion: BNP levels are an important indicator of mortality and may be useful in predicting the prognosis of patients. In addition to BNP, high troponin levels, advancing age, hypoxic and hypotensive patients, and the need for hospitalization also increase mortality, and the use of these markers will be beneficial.
